摘 要 本文对上海市徐汇区在分级诊疗方面的做法进行介绍。徐汇区区域内有丰富的医疗卫生资源，通过完善优质的社区卫生服务体系，借助信息化手段，推进慢性病长处方、医联体等试点，积极探索区域内分级诊疗可行的政策支持和技术路线。
