品管圈在提高药品盘点账物相符率中的应用(1)
摘 要 目的：提高药品盘点账物相符率，规范药品库存管理。方法：运用品管圈管理工具，统计2017年7月至2018年6月我院门诊药品盘点数据，设计影响药品账物相符原因查检表，通过现场查检，统计相关影响因素，拟定改善措施并进行效果确认。对比活动前、后药品账物相符率，计算改善幅度，对比改善效果。结果：门诊药品账物相符率由活动前的82.41%提高至93.58%，目标达成率为107.30%，改善幅度为13.55%。结论：此次品管圈活动的开展，优化了药品盘点流程，改善了患者就诊体验，降低了药品盘点耗时，为不断优化药品服务提供基础保障。

Application of quality control circle in the improvement of the account matching rate of drug inventory

ABSTRACT Objective： To improve the account matching rate of drug inventory， and standardize the management of drug inventory. Methods： The drug inventory data from July， 2016 to June， 2017 in our hospital were counted by quality control circle(QCC). The checklist of the reasons affecting drug account match was designed， the relevant influencing factors were counted by on-site inspection and the improvement measures were developed and results were confirmed. The improvement range was calculated， and the drug account matching rate and the improvement effect were compared before and after the activity. Results： The drug account matching rate in outpatient pharmacy was improved from 82.41% before the activity to 93.58% after. The target achievement rate was 107.30% and the improvement range was 13.55%. Conclusion： The drug inventory process is optimized， the patient’s experience to visit doctor is improved and the time required for drug inventory is reduced by the development of QCC， which can provide the basic guarantee for the continuous optimization of drug services.

藥品是医院流动资产的重要组成部分，药品盘点是医院药品物资管理的重点工作，对药品的质量管理和经济管理有着非常重要的作用。《三级综合医院评审标准实施细则(2011年版)》第4.15.2.3项要求：实行药品采购、贮存、供应计算机管理，药品库存量及进出量、调剂室库存量及使用量定期盘点、账物相符。实际工作中由于药品具有流通环节多、周转快、种类多等特点，加上人为因素的影响，使各医疗机构在药品库存管理工作中面临多重问题，药品账物相符率很难达到100%[1-4]。我院门诊实行电子处方，医院信息系统(HIS)实时更新药品库存，药品账物相符可以保证患者交费后顺利取药，是门诊药品调剂工作正常运行的重要支撑，极大程度上反映了药房的药品管理水平。
品管圈作为一种有效地质量管理工具，已经被广泛应用于各医疗机构的药学的管理中[5-8]。在此背景下，我院门诊西药房成立了以提高药事质量管理为目的的品管圈(quality control circle，QCC)小组，此次活动的主题定为“提高门诊药房药品盘点账物相符率”。
1 材料和方法
1.1 资料
分别收集品管圈活动前(2018年6月)和活动后(2019年1月)我院的门诊药房盘点数据，作为品管圈质量管理活动的实施依据和效果评价。
1.2 方法
成立品管圈活动小组，圈员(共10人)由门诊西药房的药师自发组成，圈长由门诊药房组长担任，调剂科主任任辅导员。此次品管圈活动依循“戴明循环”来开展，即：Plan-Do-Check-Action。Plan为活动前期计划准备；Do为对策实施与检讨；Check为效果确认、标准化处置；Action为检讨与改进。, 百拇医药(刘学辉 史香芬 吴彩霞 王淑娟 尹钊 孙志勇)
