PDCA循环管理法在中药饮片处方调剂质量管理中的应用(1)
摘 要 目的：探讨PDCA循环管理在中药饮片处方调剂质量管理中的应用。方法：随机选取医院所有科室中药处方2 680张，比较实施PDCA循环管理法前后的差错率，并分析其产生原因并提出干预措施。结果：在2 680张处方中共发现8处问题：处方无脚注、处方书写不规范、医师未签字、配伍禁忌、组方结构不清楚、漏抓或误抓药物、剂量超常和发药环节出错。PDCA循环管理法实施前差错处方75张，差错率为5.60%；实施后差错处方15张，差错率为1.12%(P<0.05)。结论：PDCA循环管理法应用于中药饮片处方调剂质量管理中，可及时发现中药处方中存在的问题，降低配药中的差错率，提高了中药饮片的调配质量，同时加强了医务工作者的业务培训和用药安全意识。

Application of PDCA cycle management in the quality management of prescription dispensation of Chinese herbal decoction pieces

ABSTRACT Objective： To explore the application of PDCA cycle management in the quality management of prescription dispensation of Chinese herbal decoction pieces. Methods： A total of 2 680 prescriptions of Chinese medicine prescribed by our hospital were randomly selected and the error rates were compared before and after implementation of the PDCA cycle management， its causes were analyzed and intervention measures were proposed. Results： A total of 8 kinds of problems were found in the 2 680 prescriptions， including no footnotes to prescriptions， irregular writing of prescriptions， no physician’s signature， incompatibility， unclear prescription structure， missed or mistakenly caught drugs， abnormal dosages and errors in the drug delivery process. There were 75 error prescriptions before implementation of PDCA cycle management and only 15 after(5.60% vs 1.12%， P<0.05). Conclusion： Application of PDCA cycle management in the quality management of prescription preparations of Chinese herbal decoction pieces can promptly find problems in the prescription of Chinese medicine， reduce the error rate in dispensation， improve the dispensing quality of Chinese medicine decoction， and strengthen the awareness of the medical training and the medication safety of medical workers.

中藥饮片处方是指医师通过望、闻、问、切等方法为患者开据的中药处方，包括中药饮片名称、数量、用法、用量等，它既是患者用药的凭证，也是医药临床应用的主要表现形式，其重要性不言而喻[1-2]。为保障中医饮片在临床中合理的使用，我院在中药饮片处方调剂质量管理中应用PDCA循环管理法，调配的差错率明显下降，且该方法可以及时发现处方中存在的问题，并有针对性地进行解决，使中药房的服务与安全质量持续提高。
1 资料与方法
1.1 一般资料
随机选取2016年5月—2019年5月九江市第一人民医院开具的中药处方2 680张，其药品限定为中药饮片，覆盖我院所有科室。其中2016年5月—2017年5月为实施PDCA循环管理法前的处方，共1 340张，2018年6月—2019年5月为实施PDCA循环管理法后的处方，共1 340张。, 百拇医药(邹红 沈冬梅 周海红)
