新型冠状病毒肺炎定点收治医院医师配置原则探讨(1)
摘要：新冠肺炎流行期间，如何配置定点医院的医疗救治力量是管理部门需要考虑的首要问题。上海市公共卫生临床中心作为上海市新冠肺炎成人患者定点收治医疗机构，严格按照“4个集中”收治原则，根据收治患者数量和疾病特点，实行分类、分层收治，配置医师力量和结构，重症和轻症患者按照医患比为2：1和1：2的原则配备医师力量，动态调配医师进出应急病房，创新医疗救治及管理模式，成立由“高级别专家组+外援专家+定点医院专家”共同构成的医疗救治模式，在有限的医疗资源中，科学、合理配备医师队伍，优化梯队建设及运行模式，多措并举，降低院内感染，提高患者救治成功率。本研究探索建立的原则对今后新发、突发传染病疫情期，医院医务人员配置及运行模式建立具有指导性意义。

Discussion on the principle of the deployment of doctors in designated hospitals for the treatment of COVID-19*

ABSTRACT：During the COVID-19 epidemic， how to deploy the medical treatment power in the designated hospitals isthe top priority for management to consider. Shanghai Public Health Clinical Center (SPHCC) is a designated medical institutionfor the treatment of adult patients with COVID-19 in Shanghai. We strictly follow the principle of "four centralization" treatment.According to the number of patients admitted and the characteristics of the disease， patients are treated in a classified andstratified basis and the medical strength and structure are adjusted. The ratio of doctor to patient with severe illness is 2： 1， and theratio of doctor to mild patients is 1：2. We dynamically adjust medical access to emergency wards and innovate medical treatmentand management modes. A medical treatment mode composed of "high-level expert group plus foreign aid experts and fixed-pointhospital experts" is established. Under our limited medical resources， we reduce hospital infections and improve the success ratesof patient treatment by scientifically and rationally arranging doctor teams. This study explores the principles established and is ofguiding significance to the distribution and operation of hospital medical personnel during the outbreak of new infectious diseasesin the future.

2019年12月以來，湖北省武汉市陆续报道了多例新型冠状病毒肺炎(简称新冠肺炎)患者，随着疫情的蔓延，中国其他省市及境外也相继出现了病例，世界卫生组织(WHO)于2020年1月30日将此次疫情定义为全球关注的公共卫生应急事件，又于3月11日描述为全球大流行[1]。新冠肺炎疫情的流行给公众健康和社会经济发展带来了巨大的影响，阻止疫情蔓延和有效救治感染者是控制疫情的主要方面。上海市公共卫生临床中心作为上海市新冠肺炎定点收治医疗机构，承担了上海市全部成人新冠肺炎患者的医疗救治任务，在短时间内面对大量新冠肺炎患者的情况下，如何实现医师资源配置最优化、团队协同最大化，提高患者救治效率和成功率，同时确保医护人员零感染，这是医院管理部门首先需要考虑的问题。本文就新冠肺炎流行期，在确保医护人员院内零感染的情况下，怎样做好新冠肺炎患者收治与医师配置平衡进行探讨。
