先天性心脏病患儿术后机械通气REE及其影响因素分析
耿博 胡宁治摘 要 目的:分析先天性心脏病患儿术后机械通气静息能量消耗(REE)及其影响因素。方法:回顾性分析本院163例先天性心脏病术后患儿的临床资料,经代谢车检测患儿术后3 h时的REE及统计学分析临床因素对患儿术后REE的影响。结果:采用间接测热法检测非蛋白呼吸商为0.81±0.23,REE实测值为(262.85±59.88)kJ/(kg·d),与Schofield公式估算值(276.99±63.25)kJ/(kg·d)比较差异无统计学意义(P>0.05)。多元逐步回归分析显示年龄与REE实测值具有负相关关系,先天性心脏病风险校正评分(RACHS-1)与REE实测值具有正相关关系(P<0.05)。术后24 h摄入能量超过REE实测值有5例(3.07%),低于REE实测值有158例(96.93%)。结论:先天性心脏病患儿术后REE的影响因素包括年龄和RACHS-1评分等,且多数患儿术后早期能量的摄入低于REE。
