摘 要 目的：探討髓内钉内固定与人工股骨头置换治疗高龄不稳定型股骨转子间骨折伴骨质疏松症的效果。方法：纳入2019年1月至2020年10月接诊的股骨转子间骨折伴骨质疏松症患者62例。按照随机数字表对照法将患者分为观察组和对照组各31例。观察组采用人工股骨头置换术治疗，对照组采用髓内钉内固定术治疗。观察两组手术相关指标、Harris评分和不良反应。结果：与对照组相比，观察组手术时间较短(P<0.05)，术中出血量较少(P<0.05)，下地负重行走时间较早(P<0.05)。观察组干预后3个月，Harris评分优良率为93.55%(29/31)，高于对照组的74.19%(23/31，P<0.05)。观察组不良反应发生率为3.23%(1/31)，低于对照组的38.71%(12/31，P<0.05)。结论：人工股骨头置换治疗高龄不稳定型股骨转子间骨折伴骨质疏松症的效果较好，手术时间短，患者术中出血量少，下地负重行走时间早，而且总优良率较好，不良反应发生率较低。
