    摘 要 目的：评价以临床药师主导的2型糖尿病患者用药宣教服务对用药依从性的干预结果，为建立社区慢病管理模式提供实践依据。方法：采取自身对照研究方法，对30例2型糖尿病患者实践规范用药宣教服务(12个月以上)，统计分析干预前后患者在用药依从性、药物认知度、血糖控制水平等方面变化情况。结果：患者用药依从性得分、药物认知度得分以及血糖达标率在干预后均得到提高(均P<0.05)。结论：规范建立以临床药师主导的药学服务利于2型糖尿病患者的长期慢病控制；未来应对接三级综合与社区医院的慢病管理模式，以提高及稳固患者用药依从性。
