    摘 要 社区主导的糖尿病多学科联合诊疗门诊是建立包括全科医师、临床药师、专职护师、中医师、公共卫生医师和检验师在内的学科团队，通过对患者实施科学、系统和全面的干预，从而达到改善预后和促进康复的目的。本文运用SWOT分析法对开展社区糖尿病多学科联合诊疗门诊进行优势、劣势、机会和风险的系统分析，为提升社区糖尿病患者的健康管理质量提供策略。
