用药接近失误的监控与防范

摘要：目的 为杜绝用药差错事件的发生，提高患者用药安全，对用药接近失误进行监控。方法 每天由药师填写报告单，每周由班组长汇总，每月上报职能部门并进行分析总结。结果 门诊药房开展PDCA后，每天接近失误发生件数由原来的23件下降到9件；通过防范措施，2012年接近失误数据接近目标值≤3‰。结论 采取适当的防范措施降低了用药接近失误的发生率，保证了患者的安全。
