摘要：目的 提高普通气管导管经鼻盲探插管的成功率。方法 择期全麻手术，筛除有经鼻插管禁忌的病例。将普通气管导管用一有韧性的导丝在头侧沿上呼吸道在头侧的投影塑形为一弧形，肾上腺素滴鼻收缩鼻腔粘膜。在快速诱导后将导管经鼻插入，导管尖端进入咽喉部后将气囊注气，接螺纹管手控呼吸，观察胸廓起伏情况来判断导管尖端的位置，再通过各种调节方法使导管尖端进入声门，最后抽空气囊，拔出导丝，将导管送入气管固定。结果 目前已完成操作的统计病例80例，其中60例成功，20例失败，盲探插管成功率为75%。结论 采用“上呼吸道投影法”可以明显提高经鼻盲探气管插管的成功率，虽然目前的成功率稍低，但经过操作者对该技术的不断熟练，成功率将会进一步提高。
