人工流产骶韧带麻醉与主韧带麻醉的临床效果对比研究(1)
摘要：目的 研究人工流产术中应用骶韧带阻滞麻醉临床效果。方法 选择2014年8月～2015年11月我院收治的106例早孕患者，并随机分成研究组(53例)与对照组(53例)，对照组给予宫颈旁主韧带麻醉，按照常规人工流产术操作。研究组给予骶韧带注射利多卡因阻滞麻醉，比较两组出血量、镇痛效果。结果 比较两组出血量，研究组、对照组平均出血量分别为22.3 ml、21.2ml，两组比较无显著差异，因而不具有统计学意义标准(P>0.05)。比较两组镇痛效果，研究组总有效率为96.2%，明显优于对照组的67.9%，具有统计学意义标准(P<0.05)。结论 在人工流产术中，骶韧带阻滞麻镇痛效果佳，副作用少，值得进一步推广。

Comparative Study on Clinical Effect of the Anesthesia of the Sacral Ligament and the Main Ligament of the Artificial Abortion

Abstract：Objective To study the clinical effect of sacral block anesthesia in induced abortion.Methods 106 cases of early pregnancy in August 2014 ～2015 year in November in our hospital， and randomly divided into study group (53 cases) and control group (53 cases)， the control group was given cervical paravertebral ligament anesthesia， according to the conventional artificial abortion operation. The study group received sacral ligament injection of lidocaine block anesthesia， compared two groups of bleeding， analgesic effect.Results The bleeding volume of the two groups was compared. The average bleeding volume of the study group and the control group was 22.3 ml and 21.2ml respectively， and there was no significant difference between the two groups， so there was no statistical significance (P>0.05). Comparing the analgesic effect of the two groups， the total effective rate of the study group was 96.2%， which was significantly better than that of the control group ()， which was statistically significant (P<0.05).Concllusion In the artificial abortion， the analgesic effect of sacral ligament anesthesia is good， and the side effects are few.

妊娠3個月内选择药物或手术方法终止妊娠，称为早期妊娠终止，多数是由于疾病不宜继续妊娠，预防遗传性或先天性畸形疾病而需终止妊娠者[1-2]。随着人们生活水平的不断提高及社会经济的快速发展，人工流产术作为计划生育及避孕失败一种重要补救措施，但多数患者难以忍受人工流产术引发的疼痛[3]。因而寻找一种良好的镇痛方法，最大程度降低患者痛苦是人工流产术的关键所在。
目前人工流产术中麻醉方法较多，其中静脉麻醉无痛人工流产术，虽然患者所受痛苦小，但是极易出现窒息、麻醉意外等副作用，而且费用高[4-5]。近2年我院人工流产术中，应用局部麻醉药利多卡因骶韧带阻滞麻醉。本研究通过回顾分析我院收治的106例早孕孕妇资料，旨在探析骶韧带阻滞麻醉应用于人工流产术中的镇痛效果，现总结报道如下。

1 资料与方法
1.1一般资料 研究选择的106例对象，均为2014年8月～2015年11月我院收治的要求终止妊娠的早孕女性患者。年龄19～45 岁，经B超检查均确诊为宫内孕，且无人工流产禁忌证。106例随机分为研究组(自愿接受利多卡因骶韧带阻滞麻醉的早孕患者)与对照组，每组各53例。对照组给予宫颈旁主韧带麻醉，研究组给予利多卡因骶韧带阻滞麻醉。比较两组孕周、年龄及孕产次等资料，无明显差异，因而不具有统计学意义标准(P>0.05)，有可比性。
1.2方法 对照组：按照常规人工流产术操作。研究组：术前告知患者排空膀胱，手术取膀胱截石位，会阴下方垫一块消毒巾，打开窥器，碘伏消毒后，在5点、7点分别注射2%利多卡因，2 ml、5 ml一次性注射器紧贴宫颈进针，入针深度大约1～2 cm，并进行回抽确定无回血，干纱布压迫穿刺点止血，扩宫器扩开宫颈，2 min后行人工流产术，按照常规人工流产术进行操作。观察两组出血量及镇痛效果。, 百拇医药(刘国旗)
