膝关节OA软骨形态改变和BML与临床表现相关性研究(1)

     摘要:目的 探讨膝关节MRI检查膝关节骨关节炎(knee joint osteoarthritis,KOA)患者膝关节软骨形态改变和骨髓水肿样病变(Bone marrow edema lesions , BML)程度与临床表现评分(Western Ontario and McMaster University Osteoarthritis Index ,WOMAC)之间相关性。方法 收集膝关节炎患者47例。采用 Philips公司Achieva 3.0T TX磁共振检查设备,8通道膝关节线圈。扫描序列包括:矢状位T2-TSE-SPAIR、冠状位T2-TSE-SPIR 、矢状位3D-FFE-WATs。图像传入Extended MR Work Space R 2.6.3.1 后处理工作站,使用全器官MR评分方法( Whole-Organ Magnetic Resonance Imaging Score ,WORMS),对膝关节软骨形态学改变及BML病变进行评估。结果 软骨形态改变WORMS评分与WOMAC正相关,相关系数r=0.41,P<0.05;BML WORMS评分与WOMAC正相关,相关系数r=0.693,P<0.05;BML 与软骨 WORMS正相关,相关系数r=0.456,P<0.05。结论 膝关节OA 患者MRI检查结果显示,软骨缺损程度、BML程度与临床表现程度呈正相关,并且BML程度与软骨缺损程度之间存在正相关。提示,软骨的缺损、BML有可能是引起KOA患者疼痛的原因,并且,两者在KOA进程中相互影响对方的发展。

    Abstract:Objective To investigate the MRI examination of knee joint osteoarthritis of the knee(knee joint osteoarthritis,KOA) in patients with knee joint cartilage morphology and bone marrow edema like lesions (Bone marrow edema lesions,BML)and the degree of clinical symptoms score (Western Ontario and McMaster University Osteoarthritis Index,WOMAC).Methods The correlation between patients with osteoarthritis of the knee 47 cases by Philips company.Achieva 3.0T TX MRI equipment,8 channel knee coil.The scanning sequences included sagittal T2-TSE-SPAIR,coronal T2-TSE-SPIR,sagittal 3D-FFE-WATs.MR Work Space R 2.6.3.1 introduced Extended postprocessing workstation, using whole organ MR scoring method (Whole-Organ Magnetic Resonance Imaging Score,WORMS),to evaluate the articular cartilage morphology and BML lesions.Results The cartilage morphology change of WORMS score and WOMAC positive,the correlation coefficient r=0.41,P<0.05;BML WORMS score and WOMAC positive correlation,correlation coefficient r=0.693,P<0.05;BML and WORMS cartilage positive correlation,correlation coefficient r=0.456,P<0.05.Conclusion The results of knee joint OA patients with MRI showed cartilage defect degree,degree and clinical manifestation of BML was positively related to the degree.And between BML and degree of cartilage defect degree has a positive correlation.The results indicated that the cartilage defect,BML may be the cause of pain in patients with KOA,and the two influence each other's development in the KOA process.

    骨关节炎(osteoarthritis,OA),也称退行性关节病、骨关节病,是由于关节软骨完整性破坏以及关节边缘软骨下骨病变,导致关节症状和体征的一组异质性疾病。并且,伴随着年龄的增长,其患病率明显增加。关节疼痛是OA重要的临床表现,长期以来人们对于引起膝关节疼痛症状的原因争论不休。MRI以其对软组织显示良好的优势,可以作为KOA退行性改变的敏感监测方法。找尋KOA患者MRI特征改变与临床症状之间的关系,为临床对KOA实施针对性治疗并减轻患者症状提供了良好的依据。, 百拇医药(陈展航 宋亭 刘祎)
