脑出血患者血清Hcy、hs—CRP、DD、FIB变化及临床意义(1)
摘要:目的 探讨脑出血患者血清同型半胱氨酸(Hcy)、超敏C-反应蛋白(hs-CRP)、D-二聚体(DD)、纤维蛋白原(FIB)水平的变化及其临床意义。方法 选取我院2015年2月~2016年10月收治的200例脑出血患者作为研究对象,依据美国国立卫生研究院卒中量表(NIHSS)评分对其病情严重程度进行分组,其中低危组61例,中危组77例,高危组62例。同时选取同期50例健康体检者作为对照。检测所有受试者的血清Hcy、hs-CRP、DD、FIB水平。结果 脑出血患者的血清Hcy、hs-CRP、DD、FIB水平均显著高于对照组(P<0.01),且随着脑出血病情严重程度的加重而明显升高(P<0.01)。经Pearson相关分析得出,血清Hcy、hs-CRP、DD、FIB水平与NIHSS评分呈显著正相关(P<0.01)。结论 脑出血患者存在明显的血清Hcy、hs-CRP、DD、FIB水平异常表现,这些血清标志物可能参与了脑出血的发病及病情进展过程。

Abstract:Objective To investigate the cerebral hemorrhage patients serum homocysteine (Hcy),high sensitive C-reactive protein(hs-CRP),two D- dimer(DD),fibrinogen(FIB)levels and clinical significance.Methods In our hospital in February 2015~2016 year in October 200 cases of cerebral hemorrhage patients as the research object,on the basis of the National Institutes of Health Stroke Scale(NIHSS)score of the severity of the disease were divided into two groups,including 61 cases of low risk group,medium risk group 77 cases,62 cases of high-risk group.At the same time,50 healthy subjects were selected as control.Detection of serum Hcy in all subjects,hs-CRP,DD,FIB.Results The serum Hcy level in patients with intracerebral hemorrhage,hs-CRP,DD,FIB were significantly higher than those in control group(P<0.01),and with the increased severity of cerebral hemorrhage and increased significantly(P<0.01).By Pearson correlation analysis, serum Hcy,hs-CRP,DD,FIB was significantly correlated with the level of NIHSS score (P<0.01).Conclusion The patients with serum Hcy,obvious cerebral hemorrhage hs-CRP,DD,FIB level of abnormal performance,these markers may be involved in the pathogenesis and progression of cerebral hemorrhage.

由于腦出血起病急骤、病情凶险且进展迅速,如不及时给予治疗易诱发急性心衰、脑疝等严重并发症,患者预后差,致残率及致死率高[1]。目前关于脑出血的发病机制仍未明确,大量研究显示,同型半胱氨酸(Hcy)、超敏C-反应蛋白(hs-CRP)、D-二聚体(DD)、纤维蛋白原(FIB)等血清标志物可能在脑卒中的发病过程中发挥了重要作用[2-3]。为进一步探讨Hcy、hs-CRP、DD、FIB这4种血清标志物与脑出血之间的关系,本研究对我院收治的200例脑出血患者与50例健康体检者分别进行了这4项血清标志物的检测,现报道如下。
1资料与方法
1.1一般资料 选取我院2015年2月~2016年10月收治的200例脑出血患者作为研究对象,所有患者均符合脑出血的诊断标准[4],并经CT及MRI检查等确诊。排除脑梗死、出凝血障碍引起的颅内出血、肝肾功能障碍、血液系统疾病等。依据美国国立卫生研究院卒中量表(NIHSS)评分对其病情严重程度进行分组,其中低危组61例,中危组77例,高危组62例。200例患者中,男117例,女83例,年龄53~84岁,平均年龄(64.6±5.3)岁,发病至入院时间0.5~32 h,平均时间(19.3±6.5)h。同时选取同期50例健康体检者作为对照组,其中男115例,女85例,年龄50~85岁,平均年龄(65.8±6.7)岁。两组性别、年龄对比无明显差异(P>0.05),具有可比性。
1.2方法 所有受试者晨起空腹抽取静脉血3 ml,分离血清后待测。采用日立7600-020生化分析仪检测血清Hcy、hs-CRP水平,其中Hcy检测采用循环酶法,hs-CRP检测采取胶乳比浊法。采用日本Sysmex-CA8000血凝仪检测血清DD、FIB水平,采取CLAUSS凝固法测定。, http://www.100md.com(李培育 王秀萍)
