左乙拉西坦单药治疗Lennox—Gastaut综合征的疗效观察(1)
摘要：目的 观察左乙拉西坦(Levetiracetam，LEV)治疗儿童难治性癫痫综合征Lennox-Gastaut (LGS)的临床效果。方法 将诊断为LGS的患儿分为左乙拉西坦治疗组(20例)与丙戊酸钠对照组(20例)，治疗组患儿给予左乙拉西坦干预，对照组患儿给予丙戊酸钠干预，治疗8 w后比较治疗组与对照组的临床疗效。比较LEV组治疗前后患儿头颅磁共振波谱、脑电图棘慢波指數的变化，同时采用中国修订韦氏儿童智力量表(WISC-R)作为评定工具，对LEV组治疗前后儿童进行智力测试及评定。结果 左乙拉西坦治疗组临床有效率为60.00%(12/20)，丙戊酸钠对照组临床有效率为30.00%(6/20)，两组病例统计学比较有显著性差异(P<0.01)。治疗组患儿头颅磁共振波谱NAA(N-乙酰天门冬氨酸)峰值升高，脑电图棘慢波指数较治疗前明显减少，患儿认知功能明显改善，差异有统计学意义(P<0.05)。结论 LEV治疗Lennox-Gastaut综合征疗效好，值得临床广泛运用。

Abstract：Objective To observe the effect of levetiracetam(Levetiracetam，LEV)in treatment of children with refractory epilepsy syndrome Lennox-Gastaut(LGS)clinical effect.Methods The diagnosis of LGS patients were divided into levetiracetam treatment group(20 cases)and sodium valproate group(20 cases)，treatment group were given levetiracetam the intervention，the control group was treated with sodium valproate intervention，the clinical curative effect after the treatment of 8 w treatment group compared with the control group.Compared with LEV group before and after treatment in children with brain magnetic resonance spectroscopy，EEG spike wave index changes，and the China Wechsler Intelligence Scale for children(WISC-R) as an assessment tool，intelligence test and evaluation of LEV group before and after treatment.Results levetiracetam treatment group clinical effective rate was 60.00%(12/20)，sodium valproate group clinical effective rate was 30.00%(6/20)，two cases were significant difference(P<0.01).The treatment group with cranial magnetic resonance spectroscopy(NAAN-acetylaspartate)peak，EEG spike wave index were significantly lower than that before treatment，cognitive function of children with significantly improved，the difference was statistically significant(P<0.05).Conclusion LEV treatment of Lennox-Gastaut syndrome has good curative effect，worthy of clinical application.
