阻塞性睡眠呼吸暂停综合征患者颈围及腰围的病情预测功能探讨(1)

     摘要：目的 阻塞性睡眠呼吸暂停综合征(OSAS) 是临床常见病、多发病、可引起和加重多种病患，尤其对心血管及代谢性疾患严重影响。通过对OSAS患者10年前后病情与颈围、腰围相关性分析，探讨OSAS患者颈围、腰围对OSAS病情的预测功能。方法 整群抽样，前后对照方法。随时间顺序随访已确诊的并可以随访到的OSAS患者1600余例，采录1630例的基本资料及10年前初诊时患心血管及代谢疾病种类，电话问询目前患心血管及代谢疾病种类。结果 OSAS患者颈围、腰围与病情在0.01水平显著相关，且与病情改变趋势在0.01水平也显著相关。结论 OSAS 腰围、颈围可以预测OSAS的病情变化及改变趋势，颈围方便易测，可能可以代替腰围评估病情。

    Predictive Function of Neck Circumference and Waist Circumference in Patients with Obstructive Sleep Apnea Syndrome

    Abstract：Objective Obstructive sleep apnea syndrome(OSAS)is a common clinical disease，frequently occurring disease，can cause and aggravate many diseases，particularly serious impact on cardiovascular and metabolic disorders.The relationship between neck circumference and waist circumference of OSAS patients and the predictive function of waist circumference on OSAS condition were analyzed by the correlation analysis of the condition and neck circumference and waist circumference before and after 10 years in OSAS patients.Methods Cluster sampling，before and after the control method.Followed up with more than 1600 cases of OSAS patients who have been diagnosed and followed up，collected 1630 cases of basic information and 10 years ago when the initial diagnosis of cardiovascular and metabolic diseases，telephone inquiries are currently suffering from cardiovascular and metabolic diseases.Results The neck circumference and waist circumference of OSAS patients were significantly correlated with the level of 0.01， and the trend was significantly correlated with the change of disease.Conclusion OSAS waist circumference and neck circumference can predict the changes of OSAS and change the trend，the neck circumference is easy to measure，may be able to replace the waist to assess the condition.

    阻塞性睡眠呼吸暫停综合征(Obstructive sleep apnea，OSAS)相关的共病：代谢性疾病，如高脂血症、糖尿病，心脑血管疾病都有很大的危害，OSAS常存在颈围偏大情况，据国内外文献报道颈围与腰围、BMI等可以预测OSAS的病情程度[1]，是否也可以预测OSAS 共病的发展趋势。各种指南已明确，腰围是腹型肥胖的评估手段，是心血管及代谢性疾病的重要预测指标。颈围与OSAS病情程度是正相关的。通过测量颈围是否能作为评估OSAS共病的手段。近些年大量研究发现颈围在许多方面优于腰围，如Onat发现，颈围对于轻度OSAHS的预测能力优于BMI、年龄、腰围、鼾症频率、鼾症响度，发现男性颈围与OSAHS的相关性更为显著，颈围诊断OSAHS的切点男性为颈围>39 cm，女性为颈围>35 cm。stabe等发现颈围与胰岛素敏感指数呈显著负相关[2]。因为颈围测量的方法简便，依从性好，准确度高。就此我们从已监测过并确诊的睡眠呼吸暂停综合征患者中回访到1600余例患者，现对这方面研究进行探讨。, http://www.100md.com(牛红丽 马利军 王海播)
