大剂量清热解毒中药汤剂治疗伤口感染的临床疗效观察(1)
摘要：目的 探讨大剂量清热解毒中药汤剂治疗伤口感染的临床疗效。方法 选取我院2013年6月～2016年6月收治的伤口感染患者80例，随机分成两组，每组40例。对照组患者单纯采用敏感抗生素治疗，观察组患者在使用抗生素基础上同时加用大剂量清热解毒中药汤剂口服治疗，比较两组临床疗效。结果 治疗后5 d和10 d，观察组创伤面积明显小于对照组，差异显著(P<0.05)；观察组患者总有效率(92.50%)明显高于对照组(75.00%)，差异显著(P<0.05)；治疗10 d后，对照组患者IL-6、IL-1β、TNF-α和TLR4表达水平明显高于观察组，差异显著(P<0.05)。结论 大剂量清热解毒中药汤剂治疗伤口感染疗效显著，能有效促进创面愈合，减轻患者炎症反应，且安全性较高，具有较高的临床应用价值。

Clinical Observation on Treatment of Wound Infection with Decoction of Chinese Herbal Medicine for Clearing away Heat and Toxic Herbs

Abstract：Objective To investigate the clinical efficacy of decoction of clearing away heat and toxic herbs to treat wound infection.Methods 80 patients with wound infection from June 2013 to June 2016 were randomly divided into two groups(n=40).The patients in the control group were treated with sensitive antibiotics，the patients in the observation group were treated with high dose of Qingrejiedu decoction on the basis of antibiotics. The clinical efficacy of the two groups was compared.Results After treatment，5 d and 10 d，the observation group wound area was significantly less than the control group，the difference was significant(P<0.05)；the total efficiency of the observation group(92.50%)was significantly higher than the control group(75.00%)，significant difference(P<0.05)；After 10 d treatment，the patients in the control group IL-6，IL-1β，TNF-α and TLR4 expression level was significantly higher than the observation group，significant difference(P<0.05).Conclusion High dose of Qingrejiedu decoction in the treatment of wound infection has obvious curative effect，can effectively promote wound healing，reduce inflammation in patients，and high safety，and has high clinical application value.

傷口感染在临床上较为常见，以局部红肿、热、痛为主要的临床症状，并伴有脓性分泌物[1]。临床上主要采用抗生素治疗伤口感染，具有一定的疗效，但长期使用效果并不理想，伤口容易产生抗药性，导致创口长时间不愈合，给患者带来极大的痛苦，并且增加了患者及整个家庭的经济负担[2-3]。临床关于中药治疗外伤感染创面的报道较多，采用中药汤剂有助于促进组织修复和创面愈合，且安全性较高[4]。本研究选取了80例在我院治疗的伤口感染患者，进一步探讨大剂量清热解毒中药汤剂治疗伤口感染的临床疗效，报告如下。
1 资料与方法
1.1一般资料
选取我院2013年6月～2016年6月收治的伤口感染患者80例，随机分成观察组和对照组，每组各40例。观察组中，男22例，女18例；年龄16～62岁，平均年龄(40.22±6.32)岁；创伤面积8～20 cm2，平均创伤面积(15.02±2.33)cm2。对照组中，男23例，女17例；年龄17～61岁，平均年龄(40.35±6.13)岁；创伤面积9～19 cm2，平均创伤面积(14.86±2.36)cm2。纳入标准：外伤伤口感染者；血源性软组织感染及化脓性关节感染者；骨科手术切口感染者；无全身感染症状；取得患者知情同意。排除标准：不能按规定疗程进行治疗，影响治疗效果者；创伤面积>20 cm2以上；创面周围有死骨和窦道形成；恶性皮肤溃疡者。两组患者一般资料方面相比较无明显差异(P>0.05)，具有可比性。, http://www.100md.com(邓金龙)
