某院门诊糖皮质激素类药物应用分析(1)
摘要：目的 调查分析我院门诊糖皮质激素类药物的使用情况，为其合理应用提供依据。方法 抽取2016年1月～6月每月1号、15号门诊处方8813张，对其中糖皮质激素类药物在各科室的使用情况，包括科室分布、患者年龄分布、药物品种使用频率等进行统计分析。结果 糖皮质激素类药物在该院门诊各科室应用广泛，占处方总数的8.31%，糖皮质激素类药物药物应用主要集中在21～60歲的成年人，占61.20%，使用频率前三名的糖皮质激素类药物分别为：地塞米松磷酸钠注射液、复方酮康唑软膏、吸入用布地奈德混悬液。结论 该院糖皮质激素类药物使用基本合理，但少数处方存在不合理用药现象，建议贯彻执行卫生部《糖皮质激素类药物临床应用指导原则》，加强合理应用糖皮质激素的监督管理。

Abstract：Objective To investigate the use of glucocorticoid drugs in our hospital，and to provide the basis for its rational application.Methods From January 2016 to June，the monthly number 1，number 15，a total of 8813 outpatient prescriptions of the glucocorticoid drugs in all sections of the application，including the distribution of departments，the age distribution of patients，statistical analysis of drug use frequency.Results Glucocorticoids are widely used in the hospital outpatient departments，accounting for 8.31% of the total number of prescription drugs， glucocorticoids application mainly in adults aged 21～60，accounting for 61.20%，the use frequency of the top three glucocorticoid drugs are： Dexamethasone Sodium Phosphate Injection，Compound Ketotonazole Cream，Budesonide Suspension for Inhalation.Conclusion The use of glucocorticoid drugs in the hospital is reasonable，but there are some cases of irrational drug use，it is recommended to implement the Ministry of Health，"glucocorticoid drugs clinical application guidelines"to strengthen the rational application of glucocorticoid supervision and management.

糖皮质激素是由肾上腺分泌的一类甾体激素，具有调节糖、脂肪和蛋白质的生物合成和代谢的作用，还具有抑制免疫、抗炎、抗毒及抗休克作用[1]。由于其在临床上的广泛应用，其不合理使用现象也随之产生。为了解本院门诊患者使用糖皮质激素类药物的使用情况，笔者对其门诊处方进行抽查并进行统计分析，现汇报如下。
1临床资料
从我院2016年1月～6月处方中抽取每月1号、15号各个科室处方，共8813张，筛选出含有糖皮质激素的处方，依据卫生部《糖皮质激素类药临床应用指导原则》与《新编药物学》(17版)，统计分析糖皮质激素类药物在各科室的使用分布情况、糖皮质激素类药使用患者的年龄和性别分布、各种糖皮质激素类药使用频率、年龄分布、联合使用与合理性等情况。

2 结果
2.1糖皮质激素类药物在各科室的分布情况
在抽取的8813张处方中，共有732张处方使用了糖皮质激素类药物，占8.31%，其在临床各个科室的分布情况见表1(其他包括血透、理疗、口腔科、妇产科等)。
2.2糖皮质激素类药物按患者年龄使用分布
糖皮质激素类药物应用主要集中在21～60岁的成年人，占61.20%，≤20岁患者占比为30.74%，≥61岁的老年患者所占比例最小，为8.06%，见表2。
2.3糖皮质激素类药物使用频率分布
通过抽查处方分析，目前我院糖皮质激素类药物注射剂以地塞米松注射液使用最多，与梅昭、倪丹红等的报道相符[2-3]。外用以皮肤科复方酮康唑软膏应用最多，吸入用布地奈德混悬液使用位居其次。以糖皮质激素类药物单用为主，2种联用有57例，未见3种联用糖皮质激素类药物，使用频率分布见表3。复方酮康唑软膏为复方制剂，每克含酮康唑10 mg和丙酸氯倍他索0.5 mg，其中的丙酸氯倍他索为强效皮质类固醇外用制剂。, 百拇医药(曹力波)
