应用无创呼吸机治疗感染合并肺间质特发性纤维化患者的临床研究(1)

     摘要:目的 探討无创呼吸机治疗感染合并肺间质特发性纤维化的临床疗效及安全性。方法 选取2012年2月~2015年6月我院收治的感染合并肺间质特发性纤维化患者76例为研究对象,其中采用常规抗感染、止咳平喘等常规治疗者40例(对照组),采用常规治疗联合无创呼吸机正压通气治疗者36例(试验组)。分析比较两组患者治疗后临床疗效及不良反应有无差别。结果 两组患者经治疗后氧分压(PaO2)和一氧化碳弥散量(DLco)较治疗前明显升高(P<0.05);心率、平均动脉压、呼吸频率较治疗前明显升高(P<0.05);且上述指标在试验组变化更为显著(P<0.05)。记录两组患者0~72 h氧分压情况,试验组PaO2升高速率显著高于对照组(P<0.05);试验组患者均能耐受72 h的无创正压通气,通气过程中发生颜面压伤1例,胃肠胀气3例。无创通气相关并发症发生率为11.10%。结论 无创呼吸机治疗感染合并肺间质特发性纤维化临床疗效确切,不良反应可耐受。

    Clinical Study of Noninvasive Ventilator in the Treatment of Infection Complicated with Pulmonary Interstitial Fibrosis

    Abstract:Objective To investigate the clinical efficacy and safety of noninvasive ventilator in the treatment of infection with pulmonary interstitial idiopathic fibrosis.Methods From February 2012~2015 June with pulmonary infection in our hospital in June were idiopathic interstitial fibrosis in 76 patients as the research object,including the use of conventional anti infection,40 cases of cough and asthma and other conventional treatment(control group),36 cases with conventional therapy combined with non invasive ventilator positive pressure ventilation treatment(experimental group)analysis. The treatment of two groups were compared the clinical efficacy and adverse reactions were no difference.Results The two groups of patients after treatment,the oxygen partial pressure(PaO2)and diffusion capacity of carbon monoxide(DLco)was significantly higher than before treatment(P<0.05);heart rate,mean arterial pressure,respiratory rate was significantly higher than that before treatment(P<0.05);and the index changes were more significant in the experimental group(P<0.05).Two groups were recorded in 0~72 h oxygen partial pressure,test group PaO2 increased rate was significantly higher than the control group(P<0.05);the experimental group were able to tolerate 72 h noninvasive positive pressure ventilation,facial pressure injury occurred in 1 cases patients,3 cases of flatulence.Noninvasive ventilation related complications the incidence rate was 11.10%.Conclusion Noninvasive ventilator is effective in the treatment of infection complicated with pulmonary interstitial fibrosis,and the adverse reactions can be tolerated.
