血脂、空腹血糖与糖化血红蛋白的关系研究(1)

     摘要:目的 探究糖化血红蛋白与空腹血糖及血脂(总甘油三酯TG、总胆固醇TC、高密度脂蛋白HDL、低密度脂蛋白LDL)的相关性。方法 将2012年1月~2105年3月收治的确诊2型糖尿病患者219例,分为糖化血红蛋白理想范围组(<7%)86例,糖化血红蛋白控制欠佳组(>7%范围)133例。以200例健康体检者为对照组。对比其空腹血糖、血脂,观察其差异性。结果 与糖化血红蛋控制欠佳组相比,糖化血红蛋白理想范围组空腹血糖明显下降(P<0.05),甘油三酯明显升高(P<0.05)。与对照组相比,糖化血红蛋白理想范围组及糖化血红蛋白控制欠佳组的空腹血糖、LDL、TG、TC明显升高,HDL降低(P<0.05)。结论 糖化血红蛋白可作为糖尿病的诊断和监控指标。糖尿病患者血脂水平与血糖控制相关,应加强对血糖与血脂的管理。

    Study on the Relationship between Blood Lipid, Fasting Blood Glucose and Glycosylated Hemoglobin

    Abstract:Objective To investigate the glycosylated hemoglobin and fasting blood glucose and blood lipid(triglyceride TG,total cholesterol TC,high density lipoprotein cholesterol HDL,low density lipoprotein LDL)correlation.Methods 219 cases of January 2012~2105 year in March were diagnosed type 2 diabetic patients were divided into the ideal range of glycated hemoglobin group(<7%)in 86 cases,a suboptimal glycated hemoglobin level group (>7%)in 133 cases.200 cases of healthy persons as control group.Compared the fasting blood glucose,blood lipid,observe its difference.Results Compared with the control group of glycosylated red blood,the fasting blood glucose in the ideal range of glycosylated hemoglobin decreased significantly(P<0.05),and triglyceride increased significantly(P<0.05).Compared with the control group,the fasting blood glucose,LDL,TG and TC of the glycosylated hemoglobin ideal range group and the glycosylated hemoglobin control group were significantly increased and the HDL was decreased(P<0.05).Conclusion Glycosylated hemoglobin can be used as a diagnostic and monitoring index for diabetes mellitus.Diabetic patients with blood glucose levels and blood glucose control should be strengthened on the management of blood glucose and blood lipids.

    糖尿病是一種以慢性血糖增高为特征的代谢性疾病,随着社会的发展及人民生活水平的提高,同时生活方式的改变,糖尿病的患病率在急速增长[1]。2型糖尿病起病隐匿,较不易被发现,较多患者在出现慢性并发症时才有临床表现。早期糖代谢异常者无典型症状,主动就医者相对较少,DM病早期诊断依然是难点[2]。目前我国采用的糖尿病诊断为空腹(FPG)、任意时间或OGTT中2 h血糖值(2hPG):①糖尿病症状(多尿、烦渴多饮和难于解释的体重减轻)加随机血糖≥11.1 mmol/L;②空腹血糖≥7 mmol/L;③OGTT 2 h血糖≥11.1 mmol/L。

    1资料与方法

    1.1一般资料

    219例患者均是我院收治的确诊2型糖尿病的住院患者,根据糖化血红蛋白水平分为糖化血红蛋白理想范围组[86例,男45例,女41例,年龄28~86岁,平均年龄(52±1.5)岁]和糖化血红蛋白控制欠佳组[133例,男68例,女67例,年龄30~75岁,平均年龄(56±2.1)岁]。对照组为200例健康体检者,排除糖尿病、高血压、冠心病、血脂异常、心、肝肾功能异常者等疾病,两组患者在基本临床资料上无明显差异性,具有可比性。, 百拇医药(周斐)
