超声乳化术治疗老年硬核白内障的疗效观察(1)
摘要：目的 探讨超声乳化术治疗老年硬核白内障患者的临床疗效。方法 选取2015年3月～2016年9月本院收治的164例(164眼)老年硬核白内障患者，根据随机原则将患者随机分为对照组和观察组，对照组82例(82眼)予以小切口白内障囊摘除术，观察组82例(82眼)予以超声乳化术，比较对照组及观察组的眼压、散光程度、视力变化情况，同时比较手术后两组患者的并发症发生率。结果 手术前，对照组及观察组的眼压、散光程度、视力比较，统计结果显示无差异，均有P>0.05。手术后，对照组及观察组的眼压均显著降低，且观察组降低得明显，对照组及观察组的视力均显著高于术前，且观察组升高得更明显；手术后观察组的角膜散光度明显小于对照组，均有P<0.05。对照组总并发症发生率为36.59%(30/82)，观察组总并发症发生率为24.39%(20/82)，经χ2检验，观察组的总并发症发生率显著低于对照组，P<0.05。结论 在老年硬核白内障患者中施以超声乳化术治疗，可有效改善患者的散光及视力，并能有效减少术后并发症的发生。

Abstract：Objective To investigate the clinical efficacy of phacoemulsification for treatment of senile cataract patients.Methods From March 2015～2016 year in September in our hospital 164 cases(164 eyes)of senile cataract patients，according to the principle of randomization were randomly divided into control group and observation group，the control group of 82 cases(82 eyes)to small incision cataract surgery，the observation group of 82 cases(82 eyes)of phacoemulsification and intraocular pressure，compared with the control group，the observation group the changes of visual acuity， astigmatism degree，and compare the two groups of patients with postoperative complications.Results Before operation，the control group and the observation group，intraocular pressure，visual acuity were compared，the degree of astigmatism，the statistical results showed no the difference was P> 0.05.After the surgery，the control group and the observation of intraocular pressure group were significantly decreased，and the observation group decreased significantly，the control group and the observation group were significantly higher than that of the vision before operation，and the observation group increased more obviously；corneal astigmatism in the observation group after operation was significantly less than that in the control group， both P<0.05.The total complication rate in the control group was 36.59%(30/82).The total complication rate in the observation group was 24.39%(20/82).By χ2 test，the total incidence of complications in observation group was significantly lower than the control group，P<0.05.Conclusion In elderly patients with cataract phacoemulsification surgery，which can effectively improve patients with astigmatism and visual acuity，and can effectively reduce the incidence of postoperative complications.

硬核白内障是指白内障核硬度为Ⅳ～Ⅴ级的白内障种类，由于该类型白内障的韧性及硬度均比较大[1-2]，且乳化所需时间也较长，手术期间产生的能量及热效应均可能导致眼部及眼部周围组织的损伤，因此手术相对来说较难，术后并發症发生率也较高[3-4]，因此在临床治疗时，选择适宜的手术方式对疾病的预后具有十分重要的意义。, 百拇医药(吴丽萍)
