BNP对CRT后期疗效的临床评价(3)
慢性心衰进程是一个渐进的过程,在这演变阶段,不但包括了神经内分泌因子,而且还包括了循环系统相关组织因子,在这诸多细胞因子共同表达上调左心室重构进而促进心力衰竭的演变进程。当心肌细胞在负荷显著增加时,例如室壁张力增加,心力衰竭时,BNP的表达明显上调。BNP的合成大部分依靠心室肌细胞完成,其主要生理功能为具有神经内分泌抑制效能,发挥其除具有利尿跟利钠之外,还具有典型的RAAS系统的拮抗功效。BNP水平较低与更多的逆重构和更好的預后相关[6]。BNP水平反映心衰严重程度,对CRT远期效果较好的预测指标[7-8]。
受试者工作特征曲线的曲线下面积在0.5以下无诊断价值,AUC在0.5~0.7时有较低的准确性,AUC在0.7~0.9时有一定的准确性,AUC在0.9以上有较高的准确性[9-10]。本研究随访6 y结果显示:植入CRT后,除了心功能NYHA分级得到明显改善之外,反映左室功能参数的相关指标也有显著提升, BNP表达水平与治疗之前相比明显下调。通过ROC曲线分析发现,CRT术后6 y患者LVESVI和BNP的的AUC均处于0.7~0.9的范围,表明其良好的预测价值。BNP作为CRT术后治疗效果评估的实验室生化参数有重要价值。此研究在通过运用超声心动图作为评价预后指标的基础上,再加以采用BNP这项参数来检测CRT术后患者的治疗效果,最终为临床工作中CRT术后的治疗效果评估系统添加了新的成员。
