降钙素原对新生儿感染性肺炎的早期诊断价值研究(1)
摘 要:目的 观察PCT在早期诊断新生儿感染性肺炎中的作用,评价其临床应用价值。方法 选择2016年7月~2017年7月在我院接受治疗的62例感染性肺炎新生儿设为肺炎组,选择同期于我院出生的60例健康新生儿设为健康组,均展开PCT检测,对比分析检测结果。结果 肺炎组新生儿PCT水平高于健康组新生儿,且肺炎组PCT阳性检出率(98.39%)高于健康组(3.33%),差异有统计学意义(P<0.05);细菌性肺炎患儿PCT阳性检出率为95.24%,高于支原体肺炎患儿PCT阳性检出率(80.00%)及病毒性肺炎患儿PCT阳性检出率(79.17%),差异有统计学意义(P<0.05);治疗后,病毒性、支原体、细菌性肺炎患儿的PCT水平及阳性检出率均有所下降,治疗前后比较差异有统计学意义(P<0.05)。结论 在感染性肺炎新生儿中,PCT有较高敏感性,展开检测可促进早期确诊率的提升,并对不同类型的肺炎进行明确,以便于尽早干预治疗。

Abstract:Objective To observe the role of PCT in the early diagnosis of neonatal infectious pneumonia,evaluation of its clinical application value.Methods 62 neonates with infectious pneumonia received treatment in our hospital from July 2016 to July 2017 were selected as pneumonia group.60 healthy newborns born in our hospital at the same time were selected as the healthy group.The PCT test was carried out and the results were compared and analyzed.Results The level of PCT in pneumonia group was higher than that in healthy group,and the positive rate of PCT in pneumonia group was(98.39%)higher than that in healthy group(3.33%),the difference was statistically significant(P<0.05).The positive rate of PCT in children with bacterial pneumonia was 95.24%.PCT positive rate in children with mycoplasma pneumonia(80.00%)and PCT positive rate in children with viral pneumonia(79.17%),the difference was statistically significant(P<0.05).After treatment,the PCT level and positive rate of children with viral,mycoplasma and bacterial pneumonia decreased,the difference before and after treatment was statistically significant(P<0.05).Conclusion PCT has a high sensitivity in neonates with infectious pneumonia,and it can promote the early diagnosis rate.Different types of pneumonia were identified to facilitate early intervention and treatment.

感染性肺炎(infectious pneumonia)是新生儿常见疾病之一,需尽早确诊并及时治疗,以确保新生儿生命安全[1]。对此类患儿展开治疗时,针对不同的发病原因,需采用不同方式,早期诊断中对感染病原菌进行明确极为关键。以往临床对病原微生物展开检测时,主要通过痰、血培养进行,但对时间、设备的要求较为严格,且检测结果容易被抗生素干扰[2]。而降钙素原(PCT)是常见炎性指标,在感染性疾病诊治中被广泛应用,近年来我院将其应用在感染性肺炎新生儿诊断中,为进一步明确其价值,本次研究对62例感染性肺炎新生儿、健康新生儿的PCT检测结果对照分析如下。
1 资料与方法
1.1一般资料 选择2016年7月~2017年7月在我院接受治疗的感染性肺炎新生儿62例,设为肺炎组,所有患儿均已明确诊断,排除吸入性肺炎、其他先天性疾病。其中男32例,女30例,日龄3~25 d,平均日龄(14.2±5.4)d,病毒感染24例,细菌性感染21例,支原体感染15例。选择同期在我院出生的健康新生儿60例,设成健康组,其中男29例,女性31例,日龄3~26 d,平均日龄(14.3±5.6)d。两组新生儿家属已对此次研究详细了解,同意参与。两组患儿性别、日龄等差异无统计学意义(P>0.05),临床可比。, 百拇医药(高艳飞)
