踝肱指数检测在住院糖尿病患者中的重复性研究(2)
本文对ABI在糖尿病患者中的檢测重复性进行了评估，结果发现该项目的重复性有待提高。有研究对脉搏波传导速度的重复性进行的多中心的评估，经过培训的技师前后两次检测肱踝脉搏波传导速度、颈股脉搏波传导速度等指标，结果发现该项目重复性良好[7]。Monti等[8]对医学生进行9 d短期培训，然后在至少有一项心血管危险因素的住院患者中进行ABI检查，发现在得到的116个ABI的结果中，医学生和专业检查人员检测结果仅有中度的一致性，阳性和阴性预测值分别为76.9%、 85.7%，阳性和因形似然比分别为6.3和3，短期培训的医学生不能胜任ABI的筛查工作。医学生的检查低估了没有PAD的患者、高估了重度PAD的患者。不过经过深入的培训后差异不再明显[9]。本文的结果提示应定期对从事该检查的人员定期进行培训，以提高检测结果的可重复性。
综上所述，本次研究纳入患者的ABI值大部分处于正常区间，未对ABI进行分层后比较前后的一致性。研究发现即使较长期从事ABI检测的人员，该项目的重复性有待提高，仍应进行定期培训。
