自制湿毒膏外治法治疗肛门湿疹的临床研究(1)
摘要：目的 观察自制湿毒膏外治法治疗肛门湿疹的临床疗效。方法 选取2014年12月～2016年12月在我院接受治疗的肛门湿疹患者86例，随机分为试验组和对照组，每组43例。试验组用自制湿毒膏外用治疗，对照组用苦参汤治疗。观察两组患者的临床症状总积分、临床疗效和复发率。结果 治疗20 d、30 d后，试验组临床症状总积分低于对照组；试验组总有效率为95.35%，高于对照组的86.05%；试验组复发率低于对照组，差异均具有统计学意义(P<0.05)。结论 应用自制湿毒膏外治法治疗肛门湿疹疗效显著，复发率低，可临床推广应用。

Clinical Study on External Treatment of Anal Eczema with Self-made Dampness Toxin Ointment

Abstract：Objective To observe the clinical effect of self-made damp plaster external treatment of anal eczema.Methods 86 patients with anal eczema in December 2014～December 2016 in our hospital were randomly divided into experimental group and control group，43 cases in each group.The experimental group was treated with self-made moist toxic cream for external use，and the control group was treated with Kushen decoction.The total score of clinical symptoms，clinical efficacy，and recurrence rate were observed in both groups of patients.Results After treatment for 20 d and 30 d，the total score of clinical symptoms in the experimental group was lower than that in the control group，and the total effective rate in the experimental group was 95.35%，which was higher than 86.05% in the control group.The recurrence rate in the experimental group was lower than that in the control group，and the difference was statistically significant(P<0.05).Conclusion Application of self-made external treatment of wet toxic plaster for the treatment of anal eczema has a significant effect，low recurrence rate，can be clinically applied.

肛門湿疹(anal eczema)是肛门周围皮肤粘膜、真皮、表皮发生炎症非感染性疾病。临床发病者多位过敏体质，属于中医学中“浸淫症”“绣球风”“风湿疡”范畴，患者主要表现为红斑、丘疹、水疱、渗出、等症状。该疾病进展快，病情容易反复，严重影响患者的健康安全[1]。现代医学较多使用糖皮质激素类等药物口服、外用，以及手术治疗，短期疗效显著，但病情容易反复发作，且有逐渐加重的趋势。随着中医药的运用与不断发展，中医药在治疗肛门湿疹一病中也得到逐渐拓展，我科经过长期的临床观察与研究总结了“自制湿毒膏”治疗方案，运用方便，且无副作用，且取得满意疗效。
1资料与方法
1.1一般资料 选取2014年12月～2016年12月在赣州市中医院接受治疗的肛门湿疹患者86例，随机分为两组，各43例。对照组男23例，女20例，年龄19～62岁，平均年龄(40.56±6.85)岁；分期：急性6例，亚急性16例，慢性21例。试验组男22例，女21例，年龄18～63岁，平均年龄(40.12±7.02)岁；分期：急性5例，亚急性18例，慢性20例。两组患者一般资料相比，差异无统计学意义(P>0.05)，有可比性。
1.2方法 ①试验组：以自制湿毒膏外用轻柔均匀涂抹30 s，2次/d。组方：青黛200 g、黄柏200 g、黄连200 g、飞滑石300 g、炉甘石300 g、五倍子150 g、蛇床子手150 g碾粉后制成膏剂(院内制作)外用涂抹。②对照组：苦参汤坐浴30 min/次，2次/d。组方：苦参、蛇床子、白芷、野菊花、黄柏、地肤子、蔺蒲、金银花各20 g煎汤制成苦参汤(院内煎制)外用坐浴。两组均以10 d为1个疗程，连用2个疗程。两组在治疗过程中忌食海鲜类、辛辣刺激性等食物，避免接触过敏物品，保持肛门部清洁卫生。, http://www.100md.com(黄德荣 张晓飞 谢沐初 刘歆 王莹 张金凤)
