宁养医疗服务对晚期癌症患者自我感受负担及应对方式的影响(1)
摘 要：目的 探讨宁养医疗服务对晚期癌症患者自我感受负担及应对方式的影响。方法 选取广元市第一人民医院于2015年10月～2016年6月收治的120例晚期癌症患者为研究对象，按就诊编号分为对照组与研究组，各60例。对照组患者采用常规护理干预，研究组在对照组的基础上采用宁养医疗服务干预。比较两组干预前后自我感受负担、应对方式及生存质量的变化情况。结果 与干预前比较，干预后研究组与对照组SPB评分均下降，且研究组下降幅度大于对照组，差异具有统计学意义(P<0.05)；与干预前比较，干预后研究组与对照组QLQ-CCC评分均上升，且研究组上升幅度大于对照组，差异有统计学意义(P<0.05)。结论 对晚期癌症患者采用宁养医疗服务进行干预，可显著改善患者自我感受负担，积极主动的面对治疗结果，提高家庭和社会在癌症患者化疗过程中的支持度，改善生存质量，具有临床推广意义。

The Effect of Ning Yang Medical Service on Self-perceived Burden and Coping Style of Patients with Advanced Cancer

Abstract：Objective To explore the effect of Ning Yang medical service on self-perceptive burden and coping style of patients with advanced cancer.Methods A total of 120 patients with advanced cancer who were admitted to Guangyuan first people's hospital in October 2015～June 2016 were selected as the research objects.They were divided into control group and research group according to the visiting number，60 cases in each group.The patients in the control group were treated with routine nursing intervention，on the basis of the control group，the study group adopted Ning Yang medical service intervention. The changes of self feeling burden， coping style and quality of life were compared before and after the intervention of the two groups.Results Compared with the pre-intervention，the SPB scores of the study group and the control group both decreased after the intervention，and the decline rate in the study group was greater than that in the control group(P<0.05).Compared with the pre-intervention，the study group and the control group after the intervention QLQ-CCC scores both increased and the increase in the study group was greater than that in the control group，the difference was statistically significant(P<0.05).Conclusion For patients with advanced cancer with hospice service intervention can significantly improve patients' self perceived burden，actively face treatment results，improve the support of family and society in the process of cancer patients during chemotherapy，improve the quality of life，has clinical significance.

隨着经济发展与疾病谱的变化，我国癌症的发病率与死亡率不断升高，严重危害人类健康，成为多发病与常见病[1]。癌症对患者的身体与心理包括整个家庭都是很强的应激源，患者癌症晚期不仅仅需要面对治疗，更要面对生命及家庭负担等[2]。癌症即恶性肿瘤，是导致人类死亡的重要原因，其具有细胞增殖及分化异常、转移性、浸润性等生物学特点，病情发生包括致癌、促癌及演变3个阶段[3]。由于大部分癌症早期无明显症状，超过80%患者确诊时已属于中晚期，失去最佳治疗时机，临床多采用放化疗及手术延长生存时间。目前，临床常采用常规护理模式对晚期癌症患者进行护理，但效果不佳。因此，采用何种方式对晚期癌症患者进行护理，是各大医院面临的难题[4]。本文选取广元市第一人民医院于2015年10月～2016年6月收治的120例晚期癌症患者为研究对象，旨在探讨宁养医疗服务对晚期癌症患者自我感受负担及应对方式的影响，为晚期癌症患者护理方案的制定提供依据，现报道如下。, http://www.100md.com(向双蓉 文俊懿)
