糖化血红蛋白测定在糖尿病诊断中的应用价值分析(1)
摘 要:目的 研究糖化血红蛋白(HbAlc)测定在糖尿病诊断中的应用。方法 选择我院2015年11月~2016年9月收治的2型糖尿病患者60例为研究组,再选择同期健康体检者60例对照组。两组入选者均于清晨抽取2 ml空腹静脉血,采用常规微粒色谱法进行测量两组入选者的HbAlc,同时对空腹血糖(FPG)与餐后2 h血糖(2hPG)进行测量。对比两组入选者的HbAlc、FPG、2hPG以及HbAlc达标率。结果 研究组HbAlc、FPG、2hPG分别为(8.89±0.67)%、(7.58±0.98)mmol/L、(12.26±2.23)mmol/L,均优于对照组的(3.56±1.21)%、(3.85±1.41)mmol/L、(7.23±2.04)mmol/L,差异有统计学意义(P<0.05)。研究组HbAlc达标率为8.33%,低于对照组的91.67%,差异有统计学意义(P<0.05)。结论 对2型糖尿病患者实施糖化血红蛋白检测,临床诊断较为可靠,诊断准确率较高,对糖尿病患者尽早治疗起到关键作用,值得临床进一步研究。

Analysis of the Value of Determination of Glycosylated Hemoglobin in the Diagnosis of Diabetes Mellitus

Abstract:Objective To study the application of glycosylated hemoglobin (HbAlc) in the diagnosis of diabetes.Methods 60 patients with type 2 diabetes admitted to our hospital from November 2015 to September 2016 were selected as the study group,and 60 healthy controls were selected as the control group.The two groups selected 2 ml of fasting venous blood in the early morning and measured the HbAlc in the two groups by conventional microparticle chromatography,and measured fasting plasma glucose(FPG)and 2 h postprandial blood glucose(2hPG).The HbAlc,FPG,2hPG,and HbAlc compliance rates were compared between the two groups. Results The HbAlc,FPG,and 2hPG in the study group were(8.89±0.67)%,(7.58±0.98)mmol/L,and(12.26±2.23)mmol/L,all of them were superior to the control group(3.56±1.21)%,(3.85±1.41)mmol/L,(7.23±2.04)mmol/L,and the difference was statistically significant(P<0.05).The HbAlc compliance rate of the study group was 8.33%,which was lower than that of the control group 91.67%,the difference was statistically significant(P<0.05).Conclusion The glycosylated hemoglobin test in patients with type 2 diabetes is more reliable in clinical diagnosis and higher in diagnostic accuracy.It plays a key role in the early treatment of diabetic patients and deserves further clinical investigation.

糖尿病(diabetes mellitus,DM)是是一組以高血糖为特征的代谢性疾病,分为1型和2型。2型糖尿病多因患者肥胖、遗传、环境等诸多原因导致,患者大多数在35~40岁以后发病,其临床表现为肥胖,导致胰岛素敏感性降低,且体内胰岛素基本完全丧失[1]。而持续的高血糖会导致患者其他器官,甚至肾、心血管衰竭,因此糖尿病需要尽早预防与治疗。而临床常规检查时,需要患者选择空腹或者早晨进行测量,时间上有一定的限制,对疾病的准确诊断造成一定影响。本文研究HbAlc测定在糖尿病诊断中的应用,旨在提高检查的准确率,现报道如下。, 百拇医药(刘爱君)
