超声检查、磁共振及血清CA125测定诊断子宫腺肌病的临床评价(1)
摘 要:目的 評价超声检查、磁共振及血清CA125测定诊断子宫腺肌病的临床价值。方法 回顾分析2016年12月~2017年12月在我院诊治的50例子宫腺肌病和子宫肌瘤患者临床资料,患者分别进行超声检查、磁共振、血清CA125测定,评价临床准确率、敏感性、特异性。结果 磁共振诊断准确率、敏感性、特异性分别为96.00%、93.10%、100.00%,高于超声检查(80.00%、75.86%、85.71%)和血清CA125测定(82.00%、72.41%、95.23%),差异有统计学意义(P<0.05)。子宫腺肌病组血清CA125水平(104.60±68.20)kU/L,高于子宫肌瘤组(22.40±16.89)kU/L,差异有统计学意义(P<0.05)。子宫腺肌病和子宫肌瘤患者临床表现(痛经、子宫增大、月经过多)较为相近,两组对比差异无统计学意义(P>0.05)。结论 超声检查、磁共振检查、血清CA125测定诊断子宫腺肌病均具有一定价值,临床可首选血清CA125测定、超声检查,对于诊断困难者进行磁共振检查,以进一步提高临床术前诊断准确率。

Abstract:Objective To evaluate the clinical value of ultrasonography,magnetic resonance imaging and serum CA125 in the diagnosis of adenomyosis.Methods The clinical data of 50 cases of adenomyosis and uterine fibroids diagnosed and treated in our hospital from December 2016 to December 2017 were retrospectively analyzed.The patients were examined with ultrasound,magnetic resonance, serum CA125,evaluate clinical accuracy,sensitivity,and specificity.Results The diagnostic accuracy, sensitivity and specificity of magnetic resonance imaging were 96.00%,93.10%,and 100.00%,respectively,which were higher than those of ultrasonography (80.00%,75.86%,85.71%)and serum CA125(82.00%,72.41%,95.23%),the difference was statistically significant(P<0.05).The serum CA125 level in the adenomyosis group was(104.60±68.20) kU/L,which was higher than that in the uterine fibroids group(22.40±16.89) kU/L,the difference was statistically significant(P<0.05).The clinical manifestations of patients with adenomyosis and uterine fibroids (dysmenorrhea,uterus enlargement,and menorrhagia)were similar,there was no significant difference between the two groups(P>0.05). Conclusion Ultrasonography, magnetic resonance imaging and serum CA125 have a certain value in the diagnosis of adenomyosis. Serum CA125 determination and ultrasonography are the first choices in clinic.Magnetic resonance imaging is used to diagnose patients with difficulty in diagnosis to further improve the preoperative diagnostic accuracy.
