舍曲林联合丙戊酸钠对癫痫伴精神障碍的疗效评价(1)
摘 要：目的 探讨舍曲林联合丙戊酸钠治疗癫痫伴精神障碍的临床疗效。方法 将2016年1月～年2017年12月湖北省黄石市第二医院接诊的70例癫痫伴精神障碍患者，按照随机数字表法分为观察组和对照组，各35例，对照组患者给予丙戊酸钠治疗，观察组患者在此基础上加用舍曲林治疗，比较两组患者疗程结束时治疗效果以及治疗前后精神障碍症状变化情况。结果 观察组治疗总有效率80.00%，高于对照组45.71%，差异有统计学意义(P<0.05)；两组患者治疗后阴性症状评分、阳性症状评分、精神病理评分，低于治疗前，差异有统计学意义(P<0.05)；观察组治疗后阴性症状评分、阳性症状评分、精神病理评分，低于对照组，差异有统计学意义(P<0.05)。结论 舍曲林联合丙戊酸钠治疗癫痫伴精神障碍疗效显著，可明显改善患者精神障碍症状。

Abstract：Objective To investigate the clinical efficacy of sertraline plus sodium valproate in the treatment of epilepsy with mental disorders.Methods From January 2016 to December 2017，the 70 patients with epilepsy with mental disorders who were admitted to the Second Hospital of Huangshi City，Hubei Province were divided into observation group and control group according to the random number table method，35 cases each，the patients in the control group were treated with sodium valproate and the patients in the observation group were treated with sertraline on this basis.The effect of treatment at the end of the treatment course and the change of mental disorders before and after treatment were compared between the two groups.Results The total effective rate of the observation group was 80.00%，which was higher than that of the control group 45.71%，the difference was statistically significant(P<0.05).The negative symptom scores，positive symptom scores，and psychopathological scores of the two groups after treatment were lower than those before treatment，the difference was statistically significant(P<0.05).The negative symptom scores， positive symptom scores and psychopathological scores in the observation group after treatment were lower than those in the control group，and the difference was statistically significant(P<0.05).Conclusion Sertraline plus valproate has a significant effect on epilepsy with mental disorders， and can significantly improve the symptoms of patients with mental disorders.

癫痫(epilepsy)是临床发病率较高的短暂性脑功能失调综合征，在患者长期病情进展过程中容易伴发精神障碍，且累及部位不同精神障碍的类型也不同，临床治疗难度较大[1]。目前临床上尚缺乏治疗癫痫伴精神障碍的特效药物，常规的丙戊酸钠、利培酮等药效有限，治愈率很低[2]。将2016年1月～2017年12月我院70例癫痫伴精神障碍患者，尝试使用舍曲林联合丙戊酸钠治疗该病获得了良好的效果，现总结报道如下。
1 资料与方法
1.1 一般资料 将2016年1月～2017年12月湖北省黄石市第二医院接诊的70例癫痫伴精神障碍患者纳入本研究。纳入标准：①经常规脑电图检查结合临床表现确诊为癫痫；②经阳性与阴性症状量表(PANSS)检测确诊为伴发精神障碍；③符合本研究用药指征；④患者家属签署知情同意书。排除标准：①情感性精神障碍或精神分裂症者；②合并其他急慢性疾病者。按照随机数字表法均分为观察组和对照组各35例，观察组男性15例，女性20例，年龄45～78岁，平均年龄(60.17±6.88)岁；对照组男性16例，女性19例，年龄45～78岁，平均年龄(60.22±6.75)歲。两组患者的性别、年龄等基线资料对比，差异无统计学意义(P>0.05)，具有可比性。, http://www.100md.com(范银燕)
