钢板内固定术联合中药煎服治疗胫骨平台骨折的临床疗效观察(1)
摘 要：目的 观察钢板内固定术联合中药治疗胫骨平台骨折的临床疗效。方法 选取我院2016年2月～2017年2月收治的96例胫骨平台骨折患者，将其随机分为观察组和对照组，各48例。观察组患者采用钢板内固定术联合中药治疗，对照组采用单纯钢板内固定术治疗，观察患者骨折愈合时间、完全负重时间、膝关节活动度、手术优良率及并发症发生情况。结果 治疗后，观察组的骨折愈合时间少于对照组[(91.05±4.36)d vs(114.39±4.08)d]、完全负重时间[(97.14±21.56)d vs(106.84±22.05)d]、6个月后膝关节活动度优于对照组，组间比较，差异均有统计学意义(P<0.05)；观察组手术优良率为89.58%，高于对照组的72.92%，差异有统计学意义(P<0.05)；对照组患者并发症发生率为31.25%，高于观察组18.75%，差异有统计学意义(P<0.05)。结论 采用钢板内固定术联合中药治疗胫骨平台骨折患者，骨折愈合时间短，优良率高，并发症少。

Abstract：Objective To observe the clinical effect of plate internal fixation combined with traditional Chinese medicine in the treatment of fracture of tibial plateau.Methods From February 2016 to February 2017，96 patients with fracture of tibial plateau were randomly divided into observation group and control group，48 cases in each group.The patients in the observation group were treated with plate internal fixation combined with traditional Chinese medicine，and the control group were treated with simple plate internal fixation.The fracture healing time，total weight bearing time，knee joint activity，the excellent and good rate of operation and the occurrence of complications were observed.Results After treatment，the healing time of fracture in the observation group was shorter than that in the control group[(91.05±4.36)d vs(114.39±4.08)d]，and the time of complete weight loading[(97.14±21.56)d vs(106.84±22.05)d]，and the knee joint motion after 6 months was better than that in the control group，the difference was statistically significant (P<0.05).The excellent and good rate of operation in the observation group was 89.58%，which was higher than that in the control group 72.92%，the difference was statistically significant(P<0.05).The incidence of complications in the control group was 31.25%，it was significantly higher than that in the observation group 18.75%，the difference was statistically significant(P<0.05).Conclusion The treatment of fracture of tibial plateau with plate internal fixation and traditional Chinese medicine has short healing time，high excellent and good rate and less complications.

脛骨平台骨折(fracture of tibial plateau)属于关节内骨折，多由膝关节受到钝性外力作用造成。由于胫骨平台是膝关节的重要负荷结构，一旦发生骨折，将会导致内、外平台受力不均，最终造成骨关节炎改变，如果对患者处理不当，直接影响膝关节功能的恢复，甚至引发其他严重并发症，如膝关节僵硬、关节炎等，还有可能留下后遗症[1]。作为关节内骨折，胫骨平台骨折多为高能量创伤所致，对膝关节功能有明显影响，其治疗一直是创伤骨科的难题。对于胫骨平台后外侧骨折及双髁骨折，传统的前外侧、前正中和双侧手术入路存在操作受限问题。近年后侧、后外侧入路已成为治疗此类高能量创伤所致骨折的新选择，然而单纯的手术治疗依然存在并发症高，愈合时间长等情况。所以，手术联合中药治疗成为研究热点。本次研究选取96例胫骨平台骨折患者观察钢板内固定术联合中药煎服治疗胫骨平台骨折的临床疗效，现报道如下。, 百拇医药(王慧明 卓春萍 王庆雷 马建华 金洪举)
