冠状动脉病变程度与血浆B型利钠肽对冠心病的影响因素(1)

     摘 要:目的 探讨冠心病患者冠状动脉病变程度与血浆B型利钠肽的关系。方法 选择我院2017年1月~8月住院的冠心病患者54例为A组,同时选择冠脉血管无狭窄的患者26例作为B组,收集所有研究对象的年龄、性别、血糖、血脂、血压、血浆脑钠肽(BNP)浓度、冠状动脉的病变部位及程度等临床资料,观察不同程度冠状动脉病变患者的血浆BNP水平。结果 血浆BNP水平A组较B组升高,统计学意义显著(P<0.001)。Logistic回归分析显示,年龄、BNP水平、舒张压是冠状动脉狭窄的独立危险因素。在A1、A2及A3组中随着Gensini评分增加,血浆BNP水平逐渐升高,Gensini评分与血浆BNP水平呈明显的正相关(r=0.594,P<0.05)。结论 血浆BNP参与了冠状动脉狭窄的病理生理过程,是冠状动脉病变的独立危险因素之一;血浆BNP的水平能一定程度上的反映冠脉病变程度。

    Influence of the Severity of Coronary Artery Disease and Plasma B Type Natriuretic Peptide on Coronary Heart Disease

    Abstract:Objective To investigate the relationship between the severity of coronary artery disease and plasma B-type natriuretic peptide in patients with coronary heart disease.Methods From January to August 2017, 54 patients with coronary heart disease were selected as group A and 26 patients without coronary artery stenosis as group B.The clinical data of age,gender,blood glucose,blood lipids,blood pressure,plasma brain natriuretic peptide(BNP)concentration,lesion location and extent of coronary artery were collected from all subjects,and plasma BNP levels were observed in patients with different degrees of coronary artery disease.Results Plasma BNP level in group A was higher than that in group B,the difference was statistically significant(P<0.001).Logistic regression analysis showed that age,BNP level and diastolic blood pressure were independent risk factors for coronary artery stenosis.In group A1,A2 and A3,the plasma BNP level increased with the increase of Gensini score.There was a positive correlation between Gensini score and plasma BNP level(r=0.594,P<0.05).Conclusion Plasma BNP is involved in the pathophysiological process of coronary artery stenosis and is one of the independent risk factors for coronary artery disease,and the level of plasma BNP can reflect the degree of coronary lesion to some extent.

    根據世界卫生组织(WHO)的统计,冠心病(coronary heart disease,CHD)是世界上最常见的死亡原因之一[1]。在我国,随着老龄化进程的加速,冠心病的发病率和死亡率近30 年来正逐步升高[2]。脑钠肽(brain natriuretic peptide,BNP)主要有利尿、利钠、扩张血管、抑制肾素、血管紧张素、醛固酮系统及交感神经系统活性等作用,并且BNP在心力衰竭中的应用已有较多的研究[3]。但冠状动脉狭窄导致心肌缺血是否与BNP的释放有关,本文就探讨冠心病患者冠状动脉狭窄程度与BNP的关系,现分析如下。, 百拇医药(卢灵锋 钟志戎 吴昆 樊岷伟)
