富血小板血浆结合自体脂肪移植在面部凹陷填充中的应用(1)
摘 要：目的 研究富血小板血浆结合自体脂肪移植在面部凹陷填充中的应用效果。方法 选取2017年3月～2018年3月在我院就诊的42例面部凹陷患者为研究对象，均采用PRP结合自体脂肪移植术治疗，观察临床填充效果及并发症情况。结果 42例患者中41例对手术效果满意，只有1例自觉改善不明显，对治疗效果不满意，手术满意度高达97.61%;1例不满意者，术后1年行二次填充;42例患者均未出现脂肪液化、感染、血肿、结节等并发症。结论 面部凹陷填充中应用PRP与自体脂肪效果理想，脂肪存活率高，无严重并发症。

Abstract：Objective To study the effect of platelet rich plasma combined with autologous fat transplantation in facial depression filling.Methods 42 patients with facial depression who were treated in our hospital from March 2017 to March 2018 were selected as the study subjects.PRP combined with autologous fat transplantation were used to observe the filling effect and complications.Results 41 of the 42 patients were satisfied with the results of the operation，only one of them was not satisfied with the results of the operation，and the satisfaction rate was 97.61%.The patient who was not satisfied accepted second filling operation 1 year after operation.No complications such as fat liquefaction，infection，hematoma，nodule were found in 42 patients.Conclusion PRP and autologous fat are effective in facial depression filling with high fat survival rate and no serious complications.

面部凹陷(facial depression)是临床常见的一种面部畸形，且以额部、颞部、面颊部居多，发生的原因较多，可能是面部发育不良、外伤、病理性等原因造成[1]。面部凹陷患者多选择手术改变外观，以满足美容整形的心理社会效应。临床常用的改善面部凹陷的材料主要有硅胶假体、透明质酸、自体脂肪等，其中假體材料因相关并发症风险大在应用中受到限制，透明质酸与自体脂肪应用较为广泛[2]。尤其是自体脂肪来源方便，安全可靠，价格低廉、塑形自然，备受广大患者的青睐。但术后吸收率不稳定，一直是当前临床应用的难题。随着不断的研究，发现自体脂肪与富血小板血浆(PRP)结合应用，可以促进脂肪移植的血管化，提高移植脂肪存活率。对此，本文作者结合临床患者，研究富血小板血浆结合自体脂肪移植在面部凹陷填充中的应用效果，现报告如下。
1资料与方法
1.1一般资料 选取2017年3月～2018年3月在天津医院整形外科就诊的42例面部凹陷患者为研究对象，本研究经过医院伦理委员会批准。纳入标准：①均符合面部凹陷诊断标准;②年龄20～44岁;③患者自愿参加本研究，并签署手术知情同意书。排除标准：①合并心、肝、肾等严重疾病者;②意识模糊的患者;③精神、智力障碍，语言表达障碍者。其中男性3例，女性39例;年龄20～44岁，平均年龄(31.20±3.11)岁;其中额部凹陷16例，颞部凹陷11例，面颊凹陷9例，眶周凹陷6例。, 百拇医药(刘文峰 顾俊平)
