VEGF与b—FGF在不同病因缺血性坏死股骨头中的表达及意义(1)
摘 要:目的 研究VEGF和b-FGF在不同病因股骨头缺血性坏死中的表达及意义,为临床针对不同病因ANFH进行个性化防治提供理论依据。方法 选择四川省骨科医院2015年1月~2018年12月因股骨头缺血性坏死行人工全髋关节置换术的股骨头为标本30例设为实验组,并按致病原因分为激素组、酒精组、特发组各10例,另选取股骨颈骨折置换的股骨头10例做为对照组,分别观察其VEGF和b-FGF的表达情况。结果 各实验组及对照组均可见VEGF及b-FGF的阳性表达。实验组阳性面积百分比均低于对照组,差异有统计学意义(P<0.05),实验组间阳性面积百分比比较,差异无统计学意义(P>0.05)。结论 股骨头缺血性坏死中VEGF与bFGF表达降低,提示细胞因子与股骨头坏死的修复有密切联系,且不同原因ANFH其致病机制可能相似。

Abstract:Objective To study the expression and significance of VEGF and b-FGF in avascular necrosis of femoral head with different etiology,and to provide a theoretical basis for clinical prevention and treatment of different causes of ANFH.Methods A total of 30 cases of femoral head with artificial total hip arthroplasty due to avascular necrosis of the femoral head were selected from the experimental group of Sichuan Provincial Orthopaedic Hospital from January 2015 to December 2018.The patients were divided into hormone group,alcohol group and idiopathic group(n=10)according to the causes of the disease,and 10 cases of femoral head replacement for femoral neck fracture were selected as control group.The expression of VEGF and b-FGF were observed separately. Results The positive expressions of VEGF and b-FGF were observed in all experimental groups and control groups.The percentage of positive area in the experimental group was lower than that in the control group,the difference was statistically significant(P<0.05). There was no significant difference in the percentage of positive area between the experimental groups(P>0.05).Conclusion The expression of VEGF and bFGF in avascular necrosis of the femoral head is decreased,suggesting that cytokines are closely related to the repair of femoral head necrosis.The pathogenesis of ANFH may be similar for different reasons.

股骨頭缺血性坏死(avascular necrosis femoral head,ANFH)是由不同病因导致血液供应被中断,引起骨组织变性、坏死,继而股骨头塌陷,最终引起髋关节退行性破坏的一种疾病[1]。常用的治疗方式包括药物治疗、髓内减压、旋转截骨、游离骨移植等,但远期效果均不理想,至晚期多会出现关节面塌陷而需要行人工全髋关节置换术[2],其术后的并发症和远期效果仍难预料。国内外一直致力于ANFH的发病机制研究,其中研究比较深入且广泛的是细胞因子。本文选取血管内皮生长因子(vascular endothellal growth factor,VEGF)和碱性成纤维细胞因子(basic fibroblast growth factor,bFGF)研究其在不同原因ANFH的表达及其意义,为临床针对不同病因股骨头坏死进行个性化防治提供理论依据。

1资料与方法
