无创双水平气道正压通气在急性呼吸窘迫综合征患者中的应用效果分析(1)

     摘要:目的 对急性呼吸窘迫综合征(ARDS)患者采取无创双水平气道正压通气治疗的应用效果进行分析。方法 选取2017年2月~2018年1月我院收治的50例ARDS患者,随机分为对照组和研究组,每组25例。对照组采取鼻塞式持续气道正压通气治疗(NCPAP),研究组采取双水平气道正压通气治疗(BiPAP),对比两组治疗前、治疗48 h的PaO2、PaCO2、SpO2指标、氧疗时间以及治疗成功率。结果 研究组患者经治疗48 h的PaO2、PaCO2、SpO2指标均优于对照组,差异具有统计学意义(P<0.05);研究组氧疗时间少于对照组[(70.35±14.33)h vs (88.79±12.63)h],治疗成功率高于对照组(96.00% vs 80.00%),差异具有统计学意义(P<0.05)。结论 采取无创双水平气道正压通气治疗ARDS患者,可显著改善患者的气血分压指标,缩短氧疗时间,提高治疗成功率。

    Abstract:Objective To analyze the application effect of non-invasive bi-level positive airway pressure therapy in patients with acute respiratory distress syndrome (ARDS). Methods A total of 50 patients with ARDS admitted to our hospital from February 2017 to January 2018 were randomly divided into control group and study group, with 25 cases in each group. The control group was treated with nasal continuous positive airway pressure (NCPAP), and the study group was treated with bi-level positive airway pressure (BiPAP). The two groups were compared with PaO2, PaCO2, and SpO2 for 48 hours before treatment. Time and treatment success rate. Results The PaO2, PaCO2 and SpO2 indexes of the study group were better than those of the control group at 48 h,the difference was statistically significant (P<0.05). The oxygen treatment time of the study group was lower than that of the control group [(70.35±14.33)h vs (88.79±12.63)h], and the success rate of treatment was higher than that of the control group (96.00% vs 80.00%),the difference was statistically significant (P<0.05). Conclusion Non-invasive bi-level positive airway pressure therapy for patients with ARDS can significantly improve the blood pressure index, shorten the oxygen therapy time and improve the treatment success rate.

    急性呼吸窘迫綜合征(acute respiratory distress syndrome,ARDS)是由肺部内、外因素所导致的临床上常见呼吸系统危重症,临床体征以顽固性低氧血症为主,患者多表现为呼吸窘迫,具有发病迅速、致死率高的特征,通过吸氧、药物控制,虽可改善窘迫症状,但整体疗效不佳[1],而进行机械通气模式治疗可降低ARDS致死率,但有创机械通气存在价格昂贵、呼吸道损伤、呼吸机相关肺部感染等问题,且属于创伤性操作,技术要求相对较高,患者有不耐机、无法讲话及进食等不足。而无创机械通气具有依从性好、操作简便等优势,可广泛应用于呼吸窘迫症的治疗中[2]。本文旨在研究ARDS患者采取无创双水平气道正压通气治疗的效果,现报道如下。, 百拇医药(徐雪峰)
