七厘散对髌骨骨折患者术后肿痛的影响(1)
摘要：目的 观察七厘散内服并外用治疗髌骨骨折术后肿痛的影响。方法 将湖南省常德市第一中医医院60例髌骨骨折内固定患者按照抽签方式随机分成治疗组和对照组，每组30例，治疗组采用口服七厘散一个疗程，并将药物外用于患处，对照组仅口服七厘散，比较关节肿胀程度、疼痛评分、临床疗效及不良反应。结果 治疗后两组患者术后VAS均下降，治疗组在第 3天、第5天和第7天疼痛度评分低于对照组(P<0.05);治疗后治疗组患肢髌骨上级、髌骨中点、腓肠肌最粗点均小于对照组，差异有统计学意义(P<0.05);治疗组的临床总有效率为96.67%，高于对照组的76.67%，差异有统计学意义(P<0.05);两组患者治疗期间无发现不良反应。结论 口服并外用七厘散能有效缓解髌骨骨折术后引起的肿痛，其疗效优于单纯内服七厘散，较安全。

Abstract：Objective To observe the effect of Qili Powder on pain and swelling after patellar fracture.Methods A total of 60 patients with patellar fracture who were treated with internal fixation in the first Hospital of traditional Chinese Medicine of Changde City， Hunan Province and randomly divided into treatment group and control group with 30 cases in each group according to the drawing of lots.In the treatment group，Qili powder was given orally for a course of treatment，and the drug was used in the affected area.In the control group，the degree of joint swelling，pain score，clinical efficacy and adverse reactions were compared.Results After treatment，VAS decreased in both groups，and the pain score in the treatment group was lower than that in the control group on the 3rd，5th and 7th day (P<0.05).After treatment， the superior patella，the middle point of patella and the thickest point of gastrocnemius in the treatment group were smaller than those in the control group，the difference was statistically significant (P<0.05).The total clinical effective rate of the treatment group was 96.67%，which was higher than that of the control group 76.67%，the difference was statistically significant (P<0.05).No adverse reactions were found during treatment in both groups.Conclusion Oral and topical Qili Powder can effectively relieve the swelling and pain caused by patellar fracture after operation，and its curative effect is better than that of Qili Powder alone， and it is safer.

清代医家谢元庆所编《良方集腋·损伤门》提出的七厘散[1]，又被称为神效七厘散。目前此药逐渐广泛用于伤科[2]，方药组成：血竭、红花、朱砂、当归、乳香、没药、麝香、冰片等，此方剂具有舒筋散瘀、活血定痛、生肌止血的功效，广泛用于各种跌打损伤，骨折。骨折术后常伴不同程度的肿胀，可导致患肢疼痛，影响术后功能锻炼、切口愈合等，重者可造成局部皮肤供血进一步障碍，使切口皮缘坏死，继而增加切口感染可能性，影响损伤组织的修复，甚至直接影响骨折的愈合。因此，尽快消除手术后的肢体肿胀，减轻疼痛，早期进行功能锻炼及功能恢复，促进骨折愈合和减少并发症均有重要意义，术后西药在治疗或缓解肿痛有明显的疗效，但存在术后存在应激性溃疡等副作用。本研究对内服并外敷七厘散治疗髌骨骨折术后所致肿痛疗效进行观察，以客观评价七厘内服散外敷的有效性及安全性，研究中医药在治疗髌骨骨折术后肿胀疼痛的独特优势，为临床探索更好治疗方法。, 百拇医药(洪学汉 段建辉)
