肌内效贴联合综合康复对脑卒中后肩手综合征的疗效观察(1)
摘要：目的 观察肌内效贴联合综合康复对脑卒中后肩手综合征的治疗效果。方法 将80例脑卒中后肩手综合征患者随机分为观察组和对照组，每组40例。对照组给予常规治疗及综合康复，观察组在对照组基础上给予肌内效贴治疗。治疗4周后，记录患者治疗前后疼痛评分(VAS)、手肿胀体积、上肢 Fugl-Meyer 运动功能评定量表(FMA)及Barthel得分情况。结果 治疗后观察组VAS评分低于对照组[(4.32±1.03)分 vs (5.78±0.91)分]，手肿胀体积低于对照组[(141.68±9.27)ml vs (177.52±8.68)ml]，Fugl-Meyer和Barthel指数评分均高于对照组[(21.42±2.18)分 vs (18.19±2.46)分，(63.89±6.98)分 vs (52.46±7.31)分]，差异有统计学意义(P<0.05)。结论 肌内效贴联合综合康复在治療脑卒中后肩手综合征中具有明显的镇痛作用，对浮肿、关节活动障碍的关节功能改善有较好疗效。

Abstract：Objective To observe the therapeutic effect of intramuscular effect combined with comprehensive rehabilitation on shoulder-hand syndrome after stroke.Methods 80 patients with post-stroke shoulder-shoulder syndrome were randomly divided into observation group and control group， with 40 cases in each group. The control group received routine treatment and comprehensive rehabilitation， and the observation group was given intramuscular effect treatment on the basis of the control group. After 4 weeks of treatment， the patient's pain score (VAS)， hand swelling volume， upper limb Fugl-Meyer motor function rating scale (FMA) and Barthel score were recorded.Results After treatment， the VAS score of the observation group was lower than that of the control group [(4.32±1.03)scores vs. (5.78±0.91)scores]， and the hand swelling volume was lower than that of the control group [(141.68±9.27)ml vs (177.52±8.68)ml].Fugl-Meyer and Barthel index scores were higher than those in the control group [(21.42±2.18)scores vs(18.19±2.46)scores，(63.89±6.98)scores vs(52.46±7.31)scores]，the difference was statistically significant(P<0.05).Conclusion Intramuscular effect combined with comprehensive rehabilitation has obvious analgesic effect in the treatment of post-stroke shoulder-hand syndrome， and has a good effect on the improvement of joint function of edema and joint activity disorder.

肩手综合征(shoulder-hand syndrome，SHS)是一种以患侧肩部疼痛性运动障碍及同侧手肿痛和肢体运动障碍为主要表现的症候群，临床上早期出现疼痛、感觉异常、血管功能障碍、水肿、出汗异常及营养障碍，后期出现手部肌肉萎缩、手指关节挛缩畸形进而导致上肢功能受限。SHS是脑卒中患者常见的并发症之一，发生率高达12.5%～70%。SHS严重限制了上肢关节活动范围，且剧烈疼痛严重影响患者康复信心，妨碍了患者的整体康复，影响患者生活质量[1]。目前临床常用的SHS保守治疗方法如针灸、肩吊带、运动疗法等常规治疗，并配合综合康复护理，有一定疗效。我科采用肌内效贴联合综合康复干预SHS取得了较好疗效，现报道如下。, 百拇医药(支小建 余海 王伟成 郑绚午 彭兴甫 钟大勇)
