维持性血液透析患者应用palindrome导管置换失功permcath导管分析(1)
摘要：目的 总结37例维持性血液透析患者palindrome导管置换失功permcath导管并发症的发生情况。方法 总结2015年1月～2016年10月我院37例维持性血液透析患者应用palindrome导管置换功能不良的permcath导管术后恢复情况及术后1年的并发症情况。37例患者均为应用permcath导管维持性血液透析患者，因导管血栓形成、透析血流量不足等原因置换为新一代palindrome导管。结果 37例患者均顺利完成手术，术后皮下隧道出血7例(18.92%)，隧道下血肿2例(5.41%)，隧道感染1例(2.70%)，导管血流量不足2例(5.41%)，导管血栓形成3例(8.11%)，导管感染1例(2.70%)，导管脱出1例(2.70%)，无气胸、神经损伤、心脏损伤、空气栓塞发生。结论 维持性血液透析患者应用palindrome导管置换失功permcath导管，手术安全有效、并发症少，术后导管功能良好，远期效果需进一步随访观察。

Abstract：Objective To summarize the complications of palindrome catheter replacement and permcath catheter complications in 37 patients with maintenance hemodialysis. Methods From January 2015 to October 2016， 37 patients with maintenance hemodialysis in our hospital were treated with perdamine catheter replacement dysfunction permcath catheter and postoperative complications.37 patients were treated with permcath catheter maintenance hemodialysis， replaced by a new generation of palindrome catheters due to catheter thrombosis， dialysis blood flow and other reasons. Results All the 37 patients underwent surgery successfully. There were 7 cases (18.92%) of subcutaneous tunnel hemorrhage， 2 cases (5.41%) of tunnel hematoma， 1 case of tunnel infection (2.70%)， and 2 cases of catheter blood flow (5.41%). There were 3 cases (8.11%) with catheter thrombosis， 1 case (2.70%) with catheter infection， and 1 case (2.70%) with catheter egress， no pneumothorax， nerve injury， heart damage， and air embolism. Conclusion The maintenance of hemodialysis patients with palindrome catheter replacement of the failed permcath catheter is safe and effective， with fewer complications. The postoperative catheter function is good， and the long-term effect needs further follow-up observation.

安全、高效的血管通路是維持性血液透析患者重要的生命线，自体血管动静脉内瘘无疑是最好的选择，但有相当一部分患者因为血管条件差、内瘘失败、心功能不全等原因不适宜动静脉内瘘成形手术。对于这类患者，半永久透析导管是目前医疗条件下比较理想的透析血管通路解决方案。既往一直采用双腔带cuff的第一代permcath导管，近年来随着技术的发展，新一代palindrome导管由于透析流量高、血液再循环率低等优点，逐渐得到推广应用。我院自2015年对维持性血液透析患者，应用palindrome半永久透析导管置换失功的permcath导管，获得良好的疗效，现报告如下。, 百拇医药(王苏杭 董春锋)
