超敏肌钙蛋白Ⅰ检测在糖尿病心肌损伤早期诊断中的价值(1)
摘要:目的 探讨在糖尿病心肌损伤的早期诊断中检测血清超敏肌钙蛋白Ⅰ(hs-cTnⅠ)的价值。方法 选取2017年1月~2019年1月本院收治的103例2型糖尿病合并心肌损伤患者为观察组,选取同期行健康体检的100例志愿者为对照组,分别检测两组血清中天门冬氨酸氨基转移酶(AST)、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、糖化血红蛋白(HbA1c)及hs-cTnⅠ水平并比较。结果 两组AST、CK、CK-MB水平比较,差异无统计学意义(P>0.05);观察组HbA1c及hs-cTnⅠ水平高于对照组,差异有统计学意义(P<0.05);在观察组患者中,HbA1c≤6.4%组与HbA1c>6.4%组的患者AST、CK及CK-MB水平比较,差异无统计学意义(P>0.05),而HbA1c≤6.4%组hs-cTnⅠ水平低于HbA1c>6.4%组,差异有统计学意义(P<0.05)。结论 针对糖尿病心肌损伤,通过对患者hs-cTnⅠ进行检测,有助于疾病的早期诊断及治疗。

Abstract:Objective To investigate the value of serum hypersensitive troponin I (hs-cTnI) in the early diagnosis of diabetic myocardial injury. Methods 103 patients with type 2 diabetes mellitus complicated with myocardial injury admitted to our hospital from January 2017 to January 2019 were enrolled in the observation group. 100 volunteers who underwent healthy physical examination at the same time were selected as the control group. The levels of acid aminotransferase (AST), creatine kinase (CK) and creatine kinase isoenzyme (CK-MB), glycated hemoglobin (HbAlc) and hs-cTnI were measured and compared.Results There were no significant differences in the levels of AST, CK and CK-MB between the two groups (P>0.05). The levels of HbA1c and hs-cTnI in the observation group were higher than those in the control group,the difference was statistically significant (P<0.05). There were no significant differences in AST, CK and CK-MB between HbAlc≤6.4% and HbA1c>6.4% in the observation group (P>0.05). The hs-cTnI of HbA1c≤6.4% was lower than that of HbA1c>6.4%, the difference was statistically significant (P<0.05).Conclusion For the diagnosis of diabetic myocardial injury, the detection of hs-cTnI in patients is helpful for the early diagnosis and treatment of the disease.

糖尿病(diabetes mellitus)是常見的慢性疾病,随着生活方式及饮食方式的改变,该病的发病率也呈现出逐年上升的趋势。临床研究表明,糖尿病的患者心肌损伤、心脏结构改变是引起心血管疾病的主要危险因素,这可能与长期高血糖有关,糖尿病患者需采取科学的方法控制血糖,以降低心血管相关疾病的发生率,改善患者预后[1]。针对糖尿病合并心肌损伤者,若要降低心肌损伤疾病对患者的影响,关键在于早期检出并发症,并予以科学的干预。心肌损伤的早期诊断通常是行血清检测,具有良好的诊断效果,但其操作复杂而逐渐被取代[2]。近几年,随着检验技术的快速发展,临床中发现将血清标志物作为心肌损伤并发症的诊断有一定的价值。本研究以佳木斯市中心医院收治的糖尿病心肌损伤患者为研究对象,观察实施hs-cTnⅠ检测对疾病诊断的价值,报告如下。, 百拇医药(王然 张军)
