中西医结合治疗小儿过敏性紫癜的疗效及对血清免疫球蛋白水平影响(1)
摘要：目的探讨犀角地黄汤加味结合酮替芬治疗小儿过敏性紫癜的疗效及对血清免疫球蛋白水平影响。方法选取2016年1月—2016年12月本院收治的小儿过敏性紫癜患儿84例，随机分为2组，对照组应用酮替芬治疗，治疗组联合应用犀角地黄汤加味治疗，对比2组患儿治疗有效率以及血清免疫球蛋白水平。结果2组患儿的临床治疗效果对比，治疗组患儿的治疗有效率显著高于对照组(P<0.05)；治疗前2组患儿的T细胞亚群各项指标无明显差异(P>0.05)，治疗后治疗组CD4+以及CD4+/CD8+显著高于对照组，CD8+显著低于对照组，差异均有统计学意义(P<0.05)；治疗前2组患儿的血清免疫球蛋白水平无明显区别(P>0.05)，治疗后治疗组IgG(免疫球蛋白G)显著高于对照组，IgM以及IgA显著低于对照组，差异均有统计学意义(P<0.05)。结论小儿过敏性紫癜在酮替芬治疗的基础上联合应用犀角地黄汤加味治疗，有利于提高治疗效果，改善血清免疫球蛋白水平。

【Abstract】Objective： To observe the curative effect of Xijiao Dihuangtang Jiawei Decoction combined with ketotifen on children with allergic purpura and its effect on serum immunoglobulin level. Methods： 84 children who come to our hospital from January， 2016 to December， 2016 were randomly divided into two groups. The control group was treated with ketotifen while the treatment group was treated with Xijiao Dihuangtang Jiawei Decoction combined with ketotifen. The curative effective rate and serum immunoglobulin levels of the two groups were compared. Results： Compared with the control group， the effective rate of the treatment group was significantly higher than that of the control group (P<0.05). Before treatment， there was no significant difference in the indicators of T cell subsets between the two groups (P>0.05). But after treatment， the levels of CD4+ and CD4+/CD8+of the test group were significantly higher than those of the control group， and CD8 was significantly lower than those of the control group and the difference was statistical significance (P<0.05). Before the treatment， there was no significant difference in level of serum immunoglobulin (P<0.05)， but after the treatment， IgG of the test group was significantly higher than that of the control group and IgM and IgA were significantly lower than those of the control group， and the difference was statistical significance (P<0.05). Conclusion： The combination of ketotifen and Xijiao Dihuangtang Jiawei Decoction in the treatment of allergic purpura children is beneficial to improve therapeutic effect and improve serum immunoglobulin level.

小兒过敏性紫癜(IHSP)作为免疫疾病的一种，临床表现主要是广泛性的血管炎症，具体的作用机制还不明确，可能同体液免疫以及细胞免疫等存在密切联系。小儿过敏性紫癜的累及范围比较广，传统上应用西医治疗作用范围比较有限，同时需要长时间治疗，毒副作用大而给治疗效果带来不良影响[1]。中医治疗该病有着靶点丰富以及毒副作用比较小，并且有利于改善免疫功能。本院在小儿过敏性紫癜治疗中，联合应用犀角地黄汤加味以及酮替芬，取得一定的临床疗效，现报道如下。, http://www.100md.com(乔海明)
