“四步法”推拿手法对腰椎间盘突出症缓解期的疗效及腰椎生物力学和生活质量的影响(1)
摘要：目的 探讨对腰椎间盘突出症(LDH)缓解期的患者，采取“四步法”推拿手法治疗的效果及对患者腰椎生物力学、生活质量的影响。方法 选取72例研究对象并随机分成治疗组与对照组，每组各36例，对照组采取单纯电针治疗，治疗组在对照组的基础上加用“四步法”推拿手法治疗，治疗10d对比2组治疗效果。结果 治疗组治疗总有效率为94.44%，显著高于对照组77.78%(P<0.05);治疗后2组PT、AP均较治疗前提高，F/E较治疗前降低，治疗组指标变化幅度较对照组显著(P<0.05);治疗后治疗组患者在生理功能、心理功能、精神健康与社会功能各生活质量评分上均显著高于对照组(P<0.05)。结论 对LDH缓解期的患者采取“四步法”推拿治疗的效果满意，有效改善患者腰椎生物力学及提高患者生活质量，值得推广。

【Abstract】Objective： To study the effect of “four-step” massage technique for patients with lumbar disc herniation(LDH) in remission and its impact on the biomechanics and quality of life of the patients' lumbar spine. Methods： 72 patients were selected as subject of study and randomly divided into a treatment group and a control group， 36 cases per group. The control group was treated with electro-acupuncture alone and the treatment group was added “four-step” massage on the basis treatment of the control group. The curative effect of the two groups was compared 10 days later. Results： The total effective rate of the treatment group was 94.44%， significantly higher than that(77.78%) of the control group(P<0.05). After treatment， the PT and AP of the two groups increased when compared with that of prior treatment， but the F/E of the two groups decreased when compared with that of prior treatment. The index changes of the treatment group were significantly higher than that of the control group(P<0.05). After treatment， the patients in the treatment group had significantly higher quality of life scores in the aspects of physiological function， psychological function， mental health and social function than the patients in the control group(P<0.05). Conclusion： The effect of “four-step” massage technique for patients in remission of LDH is satisfactory， and can effectively improve the biomechanics of the lumbar spine and improve the quality of life of the patients， worthy of wide application.

腰椎間盘突出症(Lumbar disc herniation，LDH) 是推拿科、骨科常见的疾病，流行病学显示近年来我国LDH的发病率呈逐年升高的趋势，且发病年龄也呈低龄化，这严重影响患者的生活与工作，因此需采取科学有效的方法治疗[1]。对LDH临床中主要采取手术或非手术治疗的方式，其中非手术治疗由于具有安全性及经济性特点，因此作为LDH首选的治疗方式。非手术治疗多是采取中医治疗方式，其中电针治疗是主要的方式，该治疗方式主要通过降低肌肉张力以发挥镇痛作用，然而该治疗方法的效果并不理想[2]。近年来，本院在LDH的治疗上，运用“四步法”推拿手法辅助治疗疾病，取得满意疗效，现报道如下。
