益气活血化痰方联合无创通气治疗COPD伴呼吸衰竭的临床观察(1)
摘要：目的 探討益气活血化痰方联合无创通气治疗慢性阻塞性肺疾病(COPD)伴呼吸衰竭的临床疗效。方法 将60例COPD急性发作期合并呼吸衰竭患者纳为研究对象，随机分为对照组与观察组，每组各30例。对照组予常规治疗，观察组则在对照组基础上联合中药汤剂益气活血化瘀方治疗，对比2组患者的中医症状体征积分、临床疗效、肺功能指标及动脉血气分析参数变化。结果 治疗后2组症状体征积分均低于治疗前，且观察组低于对照组，均具有显著差异(P<0.05);观察组临床总有效率86.67%高于对照组的73.33%(P<0.05);治疗后2组肺功能指标(FEV1、FEV1%及FEV1/FVC)均较治疗前改善，且观察组优于对照组，差异显著(P<0.05);治疗后2组动脉血气分析指标pH值比较差异不大(P>0.05)，但观察组中的PaO2值、PaCO2值改善明显优于对照组，差异显著(P<0.05)。结论 益气活血化痰方联合无创通气治疗COPD伴呼吸衰竭患者临床疗效较好，可有效缓解患者症状，改善肺功能及动脉血气分析指标。

Clinical Observation of Yiqi Huoxue Huatan Decoction Combined with Non-invasive Ventilation in the Treatment of COPD Patients with Respiratory Failure

【Abstract】To study the clinical effect of Yiqi Huoxue Huatan Decoction combined with non-invasive ventilation in the treatment of chronic obstructive pulmonary disease(COPD)with respiratory failure. Methods： 60 acute COPD patients with respiratory failure were included in the study， and were randomly divided into a control group and an observation group， 30 cases per group. The control group was given conventional treatment and the observation group was treated with Yiqi Huoxue Huayu Decoction on the basis treatment of the control group. The changes of TCM symptoms and signs， clinical effect， lung function indexes and arterial blood gas analysis parameters of the two groups were compared. Results： After treatment， the scores of symptoms and signs of the two groups were lower than those of the prior treatment， and the observation group was lower than the control group， both of which were significantly different(P<0.05). The total clinical effective rate of the observation group was 86.67%， higher than that of the control group， 73.33%(P<0.05). After treatment， the pulmonary function indexes(FEV1， FEV1% and FEV1/FVC)of the two groups were improved compared with those of the prior treatment and the observation group was better than the control group， and the difference was significant(P<0.05). The arterial blood gas analysis of the two groups after treatment was little difference in the index pH value(P>0.05)， but the improvement of PaO2 and PaCO2 values in the observation group was significantly better than that in the control group， and the difference was significant(P<0.05). Conclusion： Yiqi Huoxue Huatan Decoction combined with non-invasive ventilation has a good clinical effect in the treatment of COPD patients with respiratory failure， which can effectively relieve the symptoms of patients， improve their lung function and arterial blood gas analysis indicators., 百拇医药(张才来 邓声财 蓝日胜)
