经皮椎体成形术治疗胸腰椎椎体压缩骨折的近期疗效观察(1)
【摘 要】 目的 ：观察经皮椎体成形术治疗胸腰椎椎体压缩性骨折患者的近期疗效。方法： 回顾性分析91例胸腰椎椎体压缩骨折患者的病例资料，分为PVP组与保守组，其中PVP组46例，保守治疗组45例。应用VAS量表评分评估疼痛程度缓解情况，应用Oswestry功能障碍指数 [1]评估患者治疗后功能恢复情况，分别记录两组病例的入院时、24h、2周、6周、3个月、6个月的VAS评分和ODI评分，记录两组病例的骨密度、住院天数、恢复正常下地负重时间、并发症，记录治疗前、治疗后6个月Cobb角变化，统计分析两组病例的临床疗效、并发症及半年后Cobb角变化的差异性。结果： PVP组治疗后24h、2周、6周VAS评分优于保守治疗组(P<0.05)，治疗后24h、2周、6周、3个月两组间ODI评分优于保守组(P<0.05)；PVP组的并发症发生率(4.35%)低于保守治疗组(11.11%)；半年后PVP组Cobb角变化优于保守治疗组(P<0.05)。结论： PVP能够快速缓解疼痛，患者功能恢复快，手术安全性较高，并发症发生率、继发性脊柱后凸畸形发生率低于保守治疗组，近期临床疗效确切。

Abstract：Objective To observe the short-term effects of percutaneous vertebroplasty treatment of osteoporotic thoracolumbar vertebral compression fracture patients.Methods A retrospective analysis 91 cases of osteoporotic thoracolumbar vertebral compression fracture patients，divided into PVP group and conservative treatment，application visual analogue scale to assess the degree of pain relief of score，application Oswestry Disability Index to assess the recovery of function in patients after treatment，when two groups of patients were recorded on admission， 24 hours， 2 weeks， 6 weeks， 3 months， 6 months， VAS score and ODI scores，two cases of bone density recording， length of stay， time to resume normal weight-bearing ambulation， complications， recorded before treatment and after 6 months of treatment Cobb angle change，statistical analysis of the clinical efficacy of two groups of patients， the complications and six months after the change of Cobb angle difference.Results PVP group comparison with a statistically significant 24 hours after treatment， 2 weeks， 6 weeks VAS score group(P<0.05)，24 hours after treatment， 2 weeks， 6 weeks， 3 months ODI score comparison between the two groups was statistically significant(P<0.05)， the complication rate PVP group (4.35%) lower than the conservative treatment group (11.11%)， six months after the Cobb angle change PVP group comparison with conservative treatment group difference was statistically(P<0.05).Conclusion PVP can quickly relieve pain， restore function in patients with fast， high security operative complication rate， secondary kyphosis was lower than the conservative treatment group， recent clinical curative effect.

随着我国人口老龄化，老年性骨质疏松症已经成为了社会共同关注的老年健康问题。骨质疏松可引发较多并发症，其中胸腰椎椎体压缩骨折是较为严重的一种并发症 [2]。本病发病人群主要以老年人为主，多在骨质疏松基础上，存在外伤史，脊椎应力造成椎体压缩骨折。骨折发生后给患者带来难以忍受的疼痛，大部分患者发病后因疼痛、活动受限对其行动能力造成较大影响，严重影响生活质量，甚至部分患者可能丧失生活自理能力，并因长期卧床面临肺部感染、泌尿系感染、褥疮等并发症的风险。, 百拇医药(邹健 卢国睴 肖大为)
