中西医结合治疗晚期癌症相关性抑郁症30例临床观察(1)
【摘 要】 目的：观察中西医结合治疗晚期癌症相关性抑郁症的临床疗效。方法：将晚期肿瘤相关抑郁60例按入院时间随机分为对照组和治疗组，每组各30例，对照组用氟西汀胶囊治疗，治疗组以血府逐瘀汤为基础方加减联合氟西汀胶囊治疗，对比两组患者的效果，并通过汉密尔顿抑郁量表(HAMD，24项)评定患者治疗前后的抑郁情况，同时比较治疗过程中两组不良反应的发生情况。结果：治疗后治疗组HAMD减分率优于对照组，差异有统计学意义(P<0.05);两组内治疗前后比较，差异均具有统计学意义(P<0.05)。治疗过程中两组均无严重不良反应发生，差异无统计学意义(P>0.05)，治疗后治疗组患者的有效率明显优于对照组，差异具有统计学意义(P<0.05)。结论：中西医结合治疗晚期癌症相关性抑郁症具有较好的临床疗效，副作用小，值得临床推广应用。

Abstract：Objective Clinical efficacy of Xuefu Zhuyu Decoction in the treatment of the advanced carcinoma related depression. Methods the 60 patients were randomly divided into control group and treatment group according to admission time， 30 cases in the control group were treated with fluoxetine capsule， and30 cases in the treatment group were treated with Xuefu Zhuyu Decoction combined with fluoxetine capsule， The efficacy of treatment was compared between the two groups， and the depression was assessed by Hamilton Depression Scale (HAMD， 24 items) before and after treatment，and recorded the occurrence of two groups of adverse reactions in the course of treatment， to compare their differences.Results According to the HAMD depression score， the two groups were compared after treatment (P<0.05); the other two groups were compared before and after treatment (P<0.05)， the difference was significant. There was no significant difference between the two groups in the occurrence of serious adverse reactions (P>0.05) ;After treatment， the improvement of depression in the treatment group was significantly better than that in the control group， the difference was statistically significant (P<0.05).Conclusion Combination of Chinese and Western Medicine in the treatment of advanced cancer related depression has a good clinical effect and small side effects. It is worthy of clinical application.

近20年來，在环境污染、食物添加剂的大量应用以及人们生活、工作压力增加等多种的因素的综合作用下，恶性肿瘤的发病率急剧升高[1]，且多数患者在明确诊断时已为晚期。恶性肿瘤除了给患者带来巨大的躯体痛苦外，还给社会、家庭带来了巨大的经济压力。由于目前医学发展水平的限制，恶性肿瘤的治愈率仍较低，患者在诊断为癌症后，多感到生存无望，精神压力巨大，因此，癌症相关性抑郁症的发生率也较高[2]。抑郁症属于中医“郁证”范畴，笔者临床上应用血府逐瘀汤加减联合氟西汀胶囊中西医结合治疗癌症相关性抑郁症，疗效显著。现将观察效果报道如下。
1 资料与方法
1.1 一般资料 所选60例患者均为2012年10月至2016年12月在平顶山市中医院住院患者，大多数患者有生活消极，表情淡漠，害怕与外界接触等症状。随机分为两组：对照组30例，其中男13例，女17例;年龄21～62岁，平均(37.5±5.3)岁;病程2个月至4年;治疗组30例，其中男16例，女14例;年龄20～60岁，平均(38.3±4.1)岁;病程3个月至5年。两组年龄、性别、病程等一般资料比较，差异无统计学意义(P>0.05)，具有可比性。, 百拇医药(安慧娟 白明贵)
