乌司他丁联合甲强龙对急性百草枯中毒患者肺损伤保护作用的研究

摘要：目的： 目的 探讨乌司他丁联合甲强龙对急性百草枯中毒患者肺损伤的保护作用及其机制。方法： 选择我院EICU 2009年5月至2010年5月收治的急性百草枯中毒患者46例，随机分为对照组(n=23例)与治疗组(n=23例)。对照组给予常规治疗。治疗组在对照组的基础上应用乌司他丁30U,2次/天,连用10天；甲强龙500mg,1次/天,连用7天，随后减至250mg,1次/天,再连用5天。结果： 治疗组发生低氧血症、急性肺损伤、ARDS及机械通气例数少于对照组(P<0.05)；治疗组肺纹理增强、磨玻璃影、胸腔积液及肺不张者明显少于对照组(P<0.05)；治疗组发生MODS、死亡人数少于对照组(P<0.05)。结论：乌司他丁联合甲强龙治疗急性百草枯中毒可减轻肺组织损伤程度，降低患者死亡率。

Abstract:Objective:The goal discusses Ukraine to take charge of his Ding to unite the armor hadrosaurus the protective function and the mechanism which damages to the acute paraquat poison patient lung. Methods:Chooses my courtyard EICU from May,2009 the acute paraquat poison patient 46 examples which admits to May,2010,divides into the control group stochastically (the n=23 example) and the treatment group (the n=23 example).The control group gives the conventional treatment.The treatment group takes charge of his Ding in control groups foundation using Ukraine a 30U,2/day,is used together 10 day; An armor hadrosaurus 500mg,1/day,is used together 7 day,afterwards reduces to a 250mg,1/day,again is used together 5 day. Results:Treats the group to have the low oxygen blood sickness,the acute lung damage,ARDS and the machinery ventilation example number is short in the control group (P<0.05); The treatment group lung texture strengthens,the ground glass shade,the chest cavity accumulates the fluid and lung opening obviously is not short in the control group (P<0.05); The treatment group has MODS,the casualty is short in the control group (P<0.05). Conclusion:Ukraine takes charge of his Ding to unite the armor hadrosaurus treatment acute paraquat poison to be possible to reduce the lung organization extent of damage,reduces the patient mortality rate.

百草枯(Paraquat,PQ)又名克芜踪,是目前广泛使用的有机杂环类接触性除草剂，对人畜均有较强毒性。通过皮肤接触、呼吸道吸入及口服均可造成急性中毒。其主要危害是急性肺损伤,继而肺纤维化,由此导致的Ⅰ型呼吸衰竭是患者死亡的主要原因,本研究旨在探讨乌司他丁联合甲强龙对急性百草枯中毒患者肺损伤的保护作用及其机制。
1 资料与方法
1.1 研究对象。百草枯中毒患者46例，均来自我院2009年5月～2010年5月EICU住院病人。男21例,女25例,年龄12～76岁。随机分为2组,常规治疗组23例,作为对照组，其中，男10例，女13例；乌司他丁联合甲强龙治疗组23例,即试验组，其中，男9例，女15例。两组患者在性别、年龄、服毒量、服药后至就诊时间、肺部影像学表现、入院时发生急性呼吸窘迫综合征(ARDS)、多器官功能障碍综合征(MODS)比较差异均无统计学意义(见表1)。
1.2 方法。
1.2.1 治疗方法。
对照组：入院后立即给予2%碳酸氢钠溶液洗胃，直至无色、无味液体流出，并留置胃管，随即给予蒙脱石散6g加入20%甘露醇250ml胃管内灌入导泻，每6小时1次，连续5天；血液灌流(天津市阳权医疗器械有限公司生产的一次性血液灌流器 ......
