[摘要]目的：介绍局部带蒂皮瓣在唇部外伤性缺损即刻修复中的临床应用。方法：2006年5月～2011年4月，对21例外伤性唇缺损畸形患者，采用局部带蒂皮瓣进行了即刻修复者，进行临床效果观察和总结分析。结果：应用局部带蒂皮瓣，对21例唇部外伤性缺损患者，根据其不同部位和程度，分别采用了鼻唇沟皮瓣、局部推进皮瓣、“Z”形交叉皮瓣或唇红粘膜瓣、舌瓣等邻近带蒂皮瓣进行即刻修复，唇部外形恢复良好，治疗效果满意。结论：对外伤性唇缺损采用局部带蒂皮瓣进行即刻修复，因其血供良好，易取材方便、方法灵活，易于掌握，多数可一次成功，值得临床推广。

Local pedicled skin flap in labial ministry application of traumatic defect instantly repair

Abstract: Objective To introduce local pedicled skin flap in the repair of traumatic defect instantly lips clinical applications. Methods In May 2006-between April 2011,twenty-one cases of patients with traumatic lip defect,use local deformity pedicled skin flap conducted instantly repair,the clinical results were observed and summarized analysis. Results The application of local pedicled skin flap,twenty-one cases of labial ministry traumatic defect,according to its different parts of the patients were used and degree,the nasal flaps,local propulsion flap,flap or lip shape Z Red Cross double disc,tongue mucous membrane pedicled skin flap and other neighboring on immediate repair,labial ministry appearance restore good,treatment with satisfactory effect. Conclusion In this local traumatic lip defect pedicled skin flap for immediate repair,because of its blood supply is good, easy materials convenient,flexible method is easy to grasp,most can be a success,worth clinical promotion.

口唇是面部重要的组织器官之一，它具有特殊的解剖外形与生理功能。唇部外伤性缺损在颌面部外伤中时常遇到，唇部外伤后的缺损畸形不仅给患者造成饮食、语言、表情等功能障碍，而且也常给患者造成精神上的巨大痛苦。因此，患者对唇部外伤后的即刻修复和整复效果也有较高的要求。我科共收治多种原因造成的外伤性唇缺损畸形21例，根据缺损情况均采用局部带蒂皮瓣进行了即刻修复，取得了良好的临床疗效，现总结报道如下。
1 病例和方法
1.1一般资料：本组病例共收集了我科2006年5月～2011年3月收治的外伤性唇缺损畸形21例(男14例，女7例)，年龄5～55岁，平均26岁。致伤原因包括交通事故伤11例，工伤4例，斗欧伤5例，狗咬伤1例。本组病例不含可直接拉拢缝合，不会造成局部畸形的唇组织缺损。
1.2唇部外伤组织缺损类型：21例外伤性唇组织缺损患者中，按组织缺损部位可归类为上唇缺损14例，下唇缺损7例，其中合并有颌面部多发伤者15例，6两例为单纯的唇部外伤性缺损。按唇部组织结构可分为单纯皮肤缺损9例，皮肤肌肉缺损7例，全层组织缺损2例，单纯唇红组织缺损3例。
1.3手术方法
1.3.1创面处理：外伤性唇缺损因致伤原因不同，缺损的部位、深浅、范围、创面的污染程度不同，且往往合并有颌面部的其它损伤。因此，要仔细彻底的清创，对有面部其它损伤者应做好相应处理，并进行组织试对位，确定缺损的范围，因唇部血运丰富，要尽可能保留原有组织，特别是唇红组织。
1.3.2皮瓣设计：根据唇组织缺损的部位和程度，对21例患者分别采用了鼻唇沟皮瓣、局部推进皮瓣、“Z”形交叉皮瓣或唇红粘膜瓣、舌瓣等邻近带蒂皮瓣修复 ......
