[摘要]目的：探索应用自体单株毛囊移植修复上唇胡须缺损的效果。方法：对17例患者采用切取枕后安全区头皮条的方法或利用直径8mm的电动环钻获取毛发移植物，在3倍放大镜下制成单株移植物，并利用21或22G注射器针头穿刺打孔，然后用显微镊夹持毛干部，将毛发移植到缺损处以修复形态。结果：植入毛发生长良好,形态满意，移植毛发成活率在95%以上。结论：单株自体毛囊种植修复胡须缺损是一个适宜的方法。

Effect of hair autotransplantation with intact follicle on mustache loss

Abstract: Objective To explore the effect of autotransplantation of hair with intact follicle on the mustache loss on the upper lip. Methods The hair transplant was obtained by taking the scalps strips from the safe donor area or using electric trepan with diameter of 8mm in 17 patients. The hairs with intact follicle were made under 3 magnifier. The recipient area was punched with 21 or 22G syringe needle. Then the hair shaft was clamped with microforceps and the hairs were transplanted to the defective area. Results The hairs transplanted grew well. The appearance was close to the normal distribution of mustache on the upper lip, resulting in a satisfactory effect. Conclusion It is feasible to restore a mustache loss on the upper lip by hair transplantation with intact follicular in accordance with the anticipation design.

胡须是男性第二性征的一个表现，同时也是成年男性头面部重要的体表标志和美学标志。胡须缺失不仅影响容貌，而且会给患者带来沉重的心理负担。胡须修复的方法较多，但是各种方法适用的范围以及修复的效果并不一致，笔者从2010年11月～2011年10月，采用自体毛囊种植的方法对17名胡须缺损的患者进行了修复，获得了良好的效果，现报道如下。
1 临床资料
本组17例患者,年龄22～53岁,缺损的原因: 唇裂术后及外伤后唇部瘢痕9例，烧伤、烫伤5例, 先天性胡须稀疏或缺如3例。
2 手术方法
2.1 术前形态的设计：①对于部分胡须缺损者，沿原胡须轮廓设计种植形态；②对于胡须完全缺损者，遵循胡须毛发分布的特点及患者本人的要求，再根据种植技术的特点进行修改，然后与患者协商，达成一致意见后，用亚甲蓝勾画出种植的范围，2%碘酒固定。
2.2 移植物的制备：移植物的获取有2种方法：①于枕后毛发安全供区(SDA)范围内设计梭形切口，宽度为0.8 1.0cm，根据所需要的量，保留约1cm长的毛干，用含有1/100 000肾上腺素的1%利多卡因局部浸润麻醉, 切开皮肤全层；在切取头皮片时，刀片必须与毛囊生长方向保持平行, 切口的深度应达毛囊以下，包括少许皮下组织，以免破坏毛囊。电凝止血，3-0可吸收线缝合皮下，5-0尼龙线全层缝合皮肤；②毛囊单位提取技术(FUE)：用电动削发器剃除后枕部头发，常规消毒，铺巾。用含有1/100 000肾上腺素的0.5%利多卡因局部浸润麻醉，用直径8mm的电动环钻按10U/cm2的密度钻取获得移植物。
将切取的头皮或钻取的移植物置于4 ℃生理盐水纱布片上备用。于3倍放大镜下，用切割刀分离出完整的单根毛囊移植物 ......
