4.3.2直接利用额肌力量的手术：额肌瓣悬吊术是宋儒耀[29]教授首创的将额肌与其周围组织分离、制成肌瓣下牵与睑板缝合的手术。主要操作方法：于眉下切口在骨膜表面剥离额肌，掀起额肌及其表面的皮肤，再将已掀起的额肌与其表面的皮肤分离开，并于已剥离好的额肌的内侧由下向上切断，外侧也纵行切断2cm，形成额肌瓣。将肌瓣自眶隔后引入重睑切口并固定在睑板上。之后国内外多名学者对其进行了改良。周丽云等[30]利用额肌筋膜形成额肌筋膜瓣，仅做重睑切口、不做眉下切口、不打开眶隔、于眼轮匝肌深面引入额肌筋膜瓣悬吊上睑。张海明等[31]发现额肌腱膜，并以此为基础设计了额肌腱膜瓣，术中不做额肌两侧切口和额肌下剥离。此后有多位学者进行改良，但主要差别在于：⑴额肌(筋膜)瓣组织的形态：有三头[32]、叉形[33]、矩形、扇形或梯形等；⑵额肌(筋膜)瓣与上睑的固定方式：①直接通过三点固定或多点固定、褥式缝合或“8”字缝合将额肌瓣固定到睑板上；②间接固定，如：提上睑肌额肌吻合术、眼轮匝肌(或SMAS)额肌悬吊术或眶隔筋膜额肌吻合术[34]。

儿童性上睑下垂可由多种原因导致。无论何种原因所致，应当尽早对其行手术治疗，早期手术矫正上睑下垂不仅能改善患者的外观，并能预防弱视和心理发育。选择何种手术方法取决于患者上睑下垂的程度、提上睑肌功能、患者自身要求及术者的经验而定，但没有任何一种手术方法适用所有的上睑下垂矫正。
