即刻种植联合口腔正畸治疗创伤性牙缺失伴骨缺损的疗效观察(1)
[摘要]目的：研究即刻種植联合口腔正畸治疗创伤性牙缺失伴骨缺损的临床疗效及对美观效果的影响。方法：选取2012年1月至2017年1月在笔者医院接受诊治的40例创伤性牙缺失伴骨缺损患者为研究对象，根据治疗方案不同分为对照组18例、研究组22例。对照组患者接受单独的即刻种植修复治疗；研究组患者接受即刻种植修复术联合口腔正畸治疗。比较两组患者治疗前后外在美观效果变化及种植体稳定性、并发症发生率。结果：治疗后3、6个月两组患者的美观效果PES评分均高于治疗前，且研究组高于对照组，差异有统计学意义(P<0.05)；治疗后1、3、6个月研究组患者的种植体稳定性ISQ评分均高于对照组，且随着时间延长而增长，差异有统计学意义(P<0.05)；治疗后研究组患者的牙周组织溢脓、牙齿松动、牙周组织红肿发生率均略低于对照组，但差异无统计学意义(P>0.05)。结论：即刻种植修复术联合口腔正畸治疗创伤性牙缺失伴骨缺损的临床疗效显著，安全性高，能使种植体长期处于稳定状态，改善患者的牙齿咬合关系及外在美观，值得临床推广。

Clinical Observation of Immediate Implantation Combined with Orthodontics in Treating Traumatic Dental Defect with Bone Defects

Abstract： Objective To study the immediate curative effect of immediate implant combined with orthodontics in the treatment of traumatic dental loss with bone defect and its effect on aesthetic effect. Methods 40 cases of patients with traumatic dental loss and bone defect undergoing diagnosis in our hospital from January 2012 to January 2017 were selected as the research subjects. All patients were divided into two groups according to the treatment plan (18 cases in the control group， 22 cases in the study group). The patients in the control group were given immediate implant repair for treatment， and the patients in the study group were treated with immediate implant repair combined with orthodontics. Before and after treatment， the external appearance and aesthetic effect of the two groups of patients and implant stability， complication rate were observed and compared. Results The PES scores of the two groups were higher than those before treatment at 3 and 6 months after treatment， and the research group was higher than the control group (P<0.05). After 1， 3 and 6 months after treatment， ISQ scores of implant stability in the study group were higher than those in the control group， and increased with time， the difference was statistically significant (P<0.05).After the treatment， the patients in the study group had more pus， periodontal loosening and periodontal tissue infiltration in the periodontal tissues than those in the control group， but the difference was not statistically significant (P>0.05). Conclusion Immediate implants combined with oral orthodontics in the treatment of traumatic dental defects with a significant clinical effect of bone defect， high safety， long-term stability of the implant can be made to improve the patient's teeth occlusion and external beauty， worthy of clinical promotion., 百拇医药(徐巍巍 韩志梅 满一)
