瘢痕切除联合中厚皮移植整形手术对深Ⅱ度烧伤愈合后瘢痕畸形患者关节功能的影响分析(1)
[摘要]目的：探討瘢痕切除联合中厚皮移植整形手术对深Ⅱ度烧伤愈合后瘢痕畸形患者关节功能的影响分析。方法：选取2016年1月-2017年1月笔者医院接收的60例深Ⅱ度烧伤愈合后瘢痕畸形患者进行研究。按照随机数字表法，将其分成两组。对照组采用常规治疗以及功能锻炼，观察组在对照组基础上采取瘢痕切除联合中厚皮移植整形手术以及功能锻炼。比较分析两组患者的临床疗效、治疗后关节功能疗效恢复情况以及患者对治疗的满意程度。对观察组患者植皮成活率进行分析。结果：两组患者经过治疗后，观察组临床有效率为100.00%显著高于对照组的76.67%，差异有统计学意义(P<0.05)。观察组患者植皮存活植皮存活率均较高(96%以上)，其中手部、肩部植皮成活率均高于98%。术后第6、12个月观察组患者的关节功能恢复情况显著优于对照组，差异有统计学意义(P<0.05)。观察组患者对治疗的总满意率为96.67%高于对照组的76.67%，差异有统计学意义(P<0.05)。结论：瘢痕切除联合中厚皮移植整形手术对深Ⅱ度烧伤愈合后瘢痕畸形患者关节功能的恢复效果较好，患者植皮成活率较高，临床应用价值较高。

Abstrac： Objective To investigate the effect of car excision combined with moderate thickness skin grafting on joint function in patients with deep second degree burn after scar formation. Methods 60 patients with scar deformity after deep second degree burn healing in our hospital from January 2016 to January 2017 were studied. According to the random number table method， divided into two groups. The control group received routine treatment and functional exercise. The observation group received scar resection combined with middle thickness skin grafting， plastic surgery and functional exercise on the basis of the control group. The clinical efficacy of the two groups of patients， the recovery of the curative effect after the treatment and the satisfaction of the patients to the treatment were compared and analyzed. The survival rate of the patients in the observation group was analyzed. Results Two groups of patients after treatment， the clinical effective rate of the observation group was 100.00%， significantly higher than that of the control group (76.67%) ， the difference was statistically significant(P<0.05). The survival rate of skin graft and survival grafts in the observation group were all higher (96% or more)， and the survival rate of hand and shoulder grafts was higher than 98%. After 6，12 months， the recovery of joint function in the observation group was significantly better than that in the control group(P<0.05). The total satisfaction rate of the observation group was 96.67%， which was higher than that of the control group (76.67%， P<0.05).
