负压封闭引流技术联合持续冲洗治疗骨折合并皮肤软组织缺损的效果观察(1)
[摘要]目的：探討负压封闭引流技术(Vacuum sealing drainage，VSD)联合持续冲洗治疗骨折合并皮肤软组织缺损的临床效果。方法：选取笔者医院2014年4月-2016年5月收治的134例骨折合并皮肤软组织缺损患者作为研究对象，按入组先后顺序随机分为2组，每组67例。对照组采用常规手术治疗，研究组采用VSD联合持续冲洗治疗，观察两组患者治疗后临床疗效、并发症发生情况及炎症指标[白细胞计数(WBC)、C反应蛋白(CRP)、血沉(ESR)、降钙素原(PCT)、血管内皮因子(VEGF)]变化情况。结果：研究组痊愈率46.27%、总有效率89.55%，显著优于对照组的35.82%和80.60%，差异有统计学意义(P<0.05)。研究组并发症发生率显著低于对照组，差异有统计学意义(P<0.05)。两组患者治疗后WBC、CRP、ESR、PCT含量较治疗前显著下降，VEGF显著升高，且研究组显著优于对照组，差异有统计学意义(P<0.05)。研究组VAS评分、新生肉芽组织平均生长时间、伤口愈合时间、住院时间、皮肤软组织成活率均显著优于对照组，差异有统计学意义(P<0.05)。结论：VSD负压封闭引流联合持续冲洗治疗骨折合并皮肤软组织缺损效果显著，能缩短疗程，降低炎症反应，减少并发症发生。

The curative effect， correlation time， complications， such as inflammation index [white blood cell (WBC)， C reactive protein (CRP)， erythrocyte sedimentation rate (ESR)，procalcitonin (PCT) and vascular endothelial growth factor (VEGF) changes] were observed and compared between the two groups. Results The cure rate of the study group was 46.27%， and the total effective rate was 89.55%， which were significantly better than those of the control group(35.82%，80.60%)， the difference were statistically significant(P<0.05). The incidence of complication in the study group was significantly lower than that of the control group(P<0.05). After treatment， the levels of WBC， CRP， ESR and PCT in the two groups decreased significantly compared with those before treatment， while VEGF increased significantly， and the study group was significantly better than the control group(P<0.05). The VAS score， average granulation tissue growth time， wound healing time， hospitalization time and survival rate of skin and soft tissue in the study group were significantly better than those in the control group(P<0.05). Conclusion VSD negative pressure drainage combined with continuous irrigation is effective in treating fractures and skin and soft tissue defects.
