自体脂肪细胞移植隆乳术的临床效果分析(1)
[摘要]目的：探讨自体脂肪细胞及自体脂肪细胞联合不同生物材料移植隆乳的临床效果。方法：系统检索万方数据库、中国知网、维普、Pubmed上2005年-2016年已公开发表的中英文文献，同时辅以手动检索相关杂志、专业信息及网络资料，所有数据采用SPSS 22.0软件进行统计分析。结果：最终纳入15篇中英文文献(13篇中文，2篇英文)，共2 276例就医者，其中单纯自体脂肪细胞移植1 171例，自体脂肪细胞+ADSCs移植831例，自体脂肪细胞+ADSCs+PRF移植274例。结果显示，自体脂肪细胞+ADSCs组和自体脂肪细胞+ADSCs+PRF组的一次填充有效率及术后并发症发生率均优于单纯自体脂肪细胞组，差异有统计学意义(P<0.05)；但两组间比较差异无统计学意义(P>0.05)。结论：自体脂肪细胞移植隆乳时人为添加ADSCs或者PRF，不仅可提高一次填充有效率，且可减少并发症发生率，明显提高了自体脂肪移植隆乳的临床效果。

Abstract： Objective To analysize the clinical effect of autologous adipocytes and autologous adipocytes combined with different biomaterials in augmentation mammoplasty. Methods In this paper， the data were analysized by SPSS 22.0， which included the Chinese and English literatures published in Wanfang database， China Knowledge Network， Wei Pu， Pubmed from 2005 to 2016， and was supplemented by manual retrieval of relevant magazines， professional information and network data. Results A total of 2276 cases were enrolled in 15 Chinese and English literatures (13 Chinese， 2 English) ， 1171 cases were autologous adipocyte transplantation， 831 cases were autologous adipocyte+ADSCs transplantation， 274 cases were autologous adipocyte+ADSCs+PRF transplantation. The primary filling efficiency and complication rate of autologous adipocyte+ADSCs group and autologous adipocyte +ADSCs+PRF group were better than those in the simple autologous adipocyte group (P<0.05)， but there was no significant difference between the two groups (P>0.05). Conclusion Autologous adipose cells transplantation combined with ADSCs or PRF in augmentation mammoplasty can not only improve the efficiency of primary filling， but also reduce the incidence of complications， which can significantly improve the clinical effect of autologous fat transplantation augmentation mammoplasty.

近年來关于自体脂肪移植在整形美容手术中的应用越来越受到临床医生和美容就医者的喜爱，其与传统假体隆乳术相比有着无切口、无免疫排斥反应、形态真实、手感逼真的优势。自体脂肪移植是指将就医者腹部、臀部或大腿等部位多余的脂肪，经过分离纯化等步骤后，提取出其中的脂肪细胞，然后再将脂肪细胞填充到身体其他部位，现在临床上开展较多的有面部脂肪填充和胸部脂肪填充[1-2]。但临床统计调查，单纯的自体脂肪细胞填充成活率较低，并且容易发生脂肪液化、钙化、坏死等并发症，情况严重时还可能引起假性囊肿与囊性纤维化。有研究表明人为添加脂肪干细胞(Adipose-derived stem cells ， ADSCs)或者脂肪干细胞联合富含血小板纤维蛋白(Platelet rich fibrin， PRF)可以提高填充后乳房中脂肪细胞的成活率，减少重吸收，而且可减少并发症的发生[3-4]。本研究中搜集关于单纯自体脂肪细胞、自体脂肪细胞+ADSCs以及自体脂肪细胞+ADSCs+PRF的临床隆乳报道研究，分析各自的临床效果，从填充次数和并发症发生率两方面进行比较。, http://www.100md.com(郭雅倩 胡文秀 吉九威)
